Now,what to do...hip REPLACEMENT is out!I would however get it opened up and refurbished and have interviewed one person only who had it done with great results. Thanks for the confidence builder. I hav VA disability and VA medical. Hoping (praying) there is someone who can put me right. I’m almost 62 and have been in excruciating pain for almost five years now. I’m scheduled for a hip replacement but was diagnosed with psoas syndrome something in me was telling me I needed a new hip though, and finally after 5 years of being in pain and seeing 5 different doctors and getting 4 cortisone shots I have a date to get it replaced. Hearing what you said about the psoas protecting the hip made me feel a lot better that I’m doing the right thing. My MRI showed moderate to severe osteoarthritis in my left hip that’s where the pain originated and it spread to psoas and other muscles down leg so happy I didn’t listen to the first doctor and listened to my gut and came across this video

Thank you for the lecture John. It was very helpful. I do have a question though. If you had a client who was presenting this way (as a clinician who isnt capable of doing surgery), how would you treat someone if their primary problem is the hip joint? Do you take a manual therapy/joint mob approach and get them to do a bunch of general leg strengthening exercises? As a clinician myself I always want to make sure I can do more for a client than just telling them they need to get a labral surgery and sending them on their way

Can it be that the muscles are tight by themselves due to excessive sitting/overweight/lack of exercise and they are the issue without any problems at the joint itself? If yes, what is in your opinion the right way to proceed?

For example, in my case I have this muscle compensation pattern that involves the lats and the intercostalis as well. And of the lats are tight, because of their cervial inervation, they upset my neck and trapezius on the same side. Is this chain reaction something normal? I have FAI. Discovered it after 15 years of leg and back pain. The root cause is 20 years of prolonged sitting. After watching this video, I now know I am lost. My FAI is too severe to the point I cannot sit more than 30 minutes. My left knee started to spontaneously slide and my psoas tendon is extremely inflammed. My left glutes arent firing at all. I thought strengthening the glutes was the solution, yet I am totally depressed now.

Thank you for this lecture. I’m a runner with low back pathology. I have two herniations at L4-S1 for over 25 years with little problems. I’m trying to understand what’s happening with my left glute and sacrum now, as running always causes me discomfort here. I do feel that psoas is affected as well. I’ve done PT, acupuncture and chiropractic, however, I’m wondering if I should have this surgically corrected, as it literally causes my running stride to shorten at longer distances. This lecture did help me realize what the problem can be and it seems as though I’ve got several things happening at once. Originally thought to be either “runner’s butt” and/or high hamstring tendonopathy, perhaps it’s just a misfiring problem that’s causing all surrounding muscles to overload.

I'm no specialist, but I have worked my way back from an ankle surgery and a limp that I had for 20 yrs pre-surgery. The best general advice that I can share from my own experience is. While you are learning what you can and can't do right not with the femur area. Take the time to do alot of mobility and strength exercises to the areas lower then the femur, to give yourself a good healthy foundation for the femur to grow healthy to. Get the soft tissue in these areas in good health (stretching and rubber band type work outs). Think of the femur like it's the epicenter of an earthquake and slowly work your way throw the body parts that surround the femur until you get to the femur. I find this to be best for long term injury prevention due to the adjustments your body will have to make to compensate for the femur injury.. The limp will destroy other areas of the body if you don't correct it over time. Allowing my ankle to cause a limp for so long has created muscle imbalances throughout my body and its been a long process to balance the body and thus prevent further injuries to the joints and areas that were forced to take on unhealthy daily movement patterns. Your hip will be the first to change. So keep your hips healthy and mobile. Good luck.

Excellent- now is to test to see if hips are involved and how each muscle is working. ..how would you gage on a person how strong to weak a certain muscle is. Definatley good to look at the whole picture of a client- there daily activity or inactivity- habits, patterns and to obviously consider other systems in the body the nervous system, age of client, past. Yes agree overuse tight muscles etc main factors. Myofascial release. I am a massage therapist. If it was say weak glutes & tight hams - would you recommened strength for glutes and releasing hamstrings- movement would have to be relearnt to develop correct patterns. As a therapist in this case, I'd loosen off the hams- would it mean the psoas would function better if glutes worked- Bridges be a good exercise- cow cat in yoga.. Very good lecture. Thank you

5 months ago in Oct of 2022, I was involved in an auto accident that caused trauma to my right hip and lower back. I was driving and the other vehicle pulled out in front of me and my left front fender struck the right front of the other car. I was thrown abruptly into the console of my car. Within a short time I felt tightness and pain in my lower right back and hip. Later that day I could not stand or walk. Tightness and pain began to subside as days went by. So I never received treatment other than a few chiropractic sessions. I've been performing various stretches and exercises on my own. After walking on my treadmill yesterday for 35 minutes I was fine. However, today I'm experiencing a little tightness and the heel and ball of the right foot is sore. I've experienced this foot and heel pain many times since the accident. My lower leg feels a little tight. Any ideas as to a plan to completely recover on my own?

So I’ve been suffering from piriformis syndrome/sciatica for over 6 months now. Prior to getting this I used to get pain in the lower back often during/after activities. I suspect now it’s my symptoms have developed due to weak gluteal muscles and tight hipflexors (like you presented in this video). I would like to ask for your recommendations to solve this problem apart from stretching the piriformis itself which ive done for 6 months without any long lasting results. Thank you!

Great information! it gets confusing out there. I recently had a left side lying hip induction where the the leg was lifted and held in place in three different positions then pressed down by the osteopathic MD. The left leg showed quite a bit of weakness compared to the right leg.The chronic pain seems focused in the hip and radiates into the groin as well the SI joint. SI joint injection proved more diagnostic than **therapeutic as was **hoped in order to bring relief. An xray and MRI may be in order to sort this out although neither of those are totally conclusive. Needless to say he "thinks?" it may be more spine related?!? I have no numbness, tingling, etc...Where would L5 fit into all this and or some narrowing of a nerve. Had an MRI in 2018 and nothing showed up. NOT looking for a diagnosis or medical advice. Any help or suggestions appreciated. Hoping and praying to avoid and type of invasive treatments like surgery THANK YOU!!!

PLEASE advise - 4 years now of constant, persistent pain in left glute and around hip joint. Tried PT for one year before labral tear w/ FAI diagnosis and surgery in 2019. Post surgical rehab of 9 months, and now nearly 2 years post-surgery still have constant deep pain in the entire gluteal/hip region as well as lower back. I'm 34 years old now, otherwise in good physical condition, and very disciplined. Walking is the only activity that gives some relative pain relief (sitting, lying, standing, weight bearing cause pain). Do you know why the injury may be so resistant? What do you suggest? (PS - I had a top rated surgeon from USC, Dr. Weber, and have tried 3 different PT clinics). Thank you!

So informative!! Question i had surgeory on both hips for labrum tear. Now after 9 years back in pain now for 7 months left hip. Arthro mri shows nothing major (no new labrum tear) pain i have is mainly hamstring and inside thigh under groin. And sometimes click in groin (painless click) Cortisone shot did nothing to relieve pain. Could pain come from lower back prblm (two disc bulges l4 l5- l5 s1) or is it most probably hip joint related?

Probably still from the hip the joint if honest, a normal MRI might not see the labrum if torn, maybe look at a MR Arthrogram as the Gold standard. The lower 2 discs can also refer pain but I think it is more than likely the hip joint that is responsible, hope that helps, regards JG

Obviously if tight psoas is the cause, releasing it will be a crucial part of the treatment but also if the movement pattern has been adapted for long enough, there will be maladaptive biomechanics for which I assume you will have to re-educate the patient's brain to the correct muscle recruitment / firing pattern? My brother had a problems lifting the right leg, was limping and had pain the right hip area for which doctors performed a surgery of psoas muscle abscess which was diagnosed to spinal tuberculosis on April 2022, since few days back his pain recurred. Why would the pain might occur if their is no abscess present in the spine Or in the psoas muscle. And the mri also shows no presence of abscess.

Excellent presentation… I’ve been unfortunate enough to fracture both hips in my younger years. Fast forward to 52 and my right psoas is killing - it started after Murph 2 yrs ago and I just can’t heal it. I’ve done acupuncture, PT several times and chiropractic. It’s gotten so “tight”- a high pitched pain like it will “pop”- that I have trouble even sitting Indian style. I did pull that hamstring about 5 yrs ago and now this. I just am not sure what in the world it could be. I’ve seen an orthopedic and he says I have an impingement but did seem overly concerned…. Any thoughts?🤔 thank you!

And if all the problems are caused by a FAI, what are you suggesting to do? Strengthen hip flexors? Because a surgery to take off FAI isn't an option (the impingement can grow again; the surgery is pretty invasive/risky, etc.). Or something else?

I've been living with hip pain for the past 6 years. I saw a doctor once but he did nothing to help. Same with the chiropractor. I've been managing myself by stretching, but recently, I've been getting worse lower back pain and my left glute is very weak now. Just hearing you go through this is bringing a tear to my eye. I feel like I finally found someone that might understand the pain that I'm going through. Please, how do I get treatment for this? Can it be fixed? It's completely destroying my quality of life.

See a sport physical therapist. I would also recommend doing yoga twice a week. And get on a total body workout regimen with glutes emphasis at-least twice a week.

It could indeed as the psoas is antagonist and if the hamstrings are short and tight the motion of hip flexion is more difficult because you have a resistance posteriorly - especially running etc, regards JG

If I were to go into the front splits where my right leg is stretched in front and my back leg is stretched 90 degrees backwards is this bad for the ligaments or tendons connecting the femor with the hip capsule? Is it possible to only stretch the muscles to extend the hip back without it affecting the ligaments and tendons? I'm not sure if it's bad for my body to learn the front splits idk if its hyperextending the ligaments?

Thanks for the message, that movement you mention for a lot of people can be normal (not for me though as so inflexible) and its not a problem, the movement will naturally stretch the ligaments and tendons etc so personally wouldn't worry to much about it, unless it becomes painful, then i would stop doing that motion....regards JG

@@JohnGibbons Thank you for the reply. Would it weaken the ligaments and tendons over time though? It sounds possible but doesnt seem healthy. Just because one can do it doesnt make it a good thing. Is it possible to only stretch the muscle without the ligaments being stretched. It sounds like the illiofemoral ligaments are only made to go back 45 degrees for normal range of motion but I could be wrong.

Hi John my son muscles are tight & stiff at the back of his knees he cannot stretch out his legs all the way, what can I do to release his muscles

Utilise the skill set of a decent soft tissue therapist is the best option - if he is a teenager then wouldn't stretch him much as he will be going through growth spurts so the long bones grow (femur etc) and the muscles play 'catch up' like the hamstrings so they feel 'tight' but are not short they are on 'stretch' so not much stretching even though they feel like its needed. regards JG

Really enjoyed this presentation.. THANK YOU. So how would we measure the weakness of the glutes... Would we ask the patient to abduct their hip and measure using Oxford grading scale, or possibly palpate to feel muscle bulk.

Thank you for providing this video! I’ve had hip/knee/groin/buttock/abdominal pain for 10 months. From watching this video, im sure it’s related to a weak glut muscle as well as my psoas. What type of doctor would you recommend to address this issue? I am looking for ways to fix the problem. Wondering if squats will help. The burning in my hip, groin and abdomen is so frustrating.

What about having flat feet. Been trying to do hip flexor strength exercises and glute exercises, psoas has been tight for a while. I'm a cleaner and been naughty lately not wearing the best shoes. Never had this tight posas and glute for this long.

Hi, could a lazy glute cause chronic low back pain one sided, shooting pains in SI joint on the same side , tigh psoas again on the same side.. Pain in groin and snapping hip, snapping knee.. and intermittent ache under front ribs again.. On the same side. These are all the symptoms I have and chiropractor mentioned a dysfunction in the SI joint and tight psoas..was hyperlordosis but this has been manipulated now but still muscle issues imbalances... I know my glutes are lazy.. I have a desk job.. Xrays look ok apart from mild scoliosis. Advice would be much appreciated, I've become so tired of pain. Thank you

Sounds like the gluteals are involved, however you have to find the underlying cause of the problem first as the inactive glutei might just be a symptom. The video looks at the hip which is a major cause of the symptoms you are saying, regards JG

can't pull left leg over right leg but can put right leg goes over left had knee replacement one spot on outside left hip nerve hurts all way down into knee and in front of FEMA bottom ley muscle in front of the top of leg can hip spurs cause large nerve shoot down I worked in cotton mill pulling one way with the left arm used one side for 30 years 12 hours a day had a knee replacement but at night before bone on bone on knee had spurs on knee all right sides of both knee and hip on the right side good hip wore down with spurs nerve pain and knee joint wore out same time knee hurts with muscles to tight all from hip to leg hurt and nerve pain do I need hip tested in all ways what test do I take the bottom of back vertebrates touching on one side.it is the hip I had problems before could not sleep hip jerking had to rub have 65 degrees curves in the backbone do I have all test made of hip and see my knee replacment in middle of new knee bouncing component to lose.

Question! Ugh I wish I could see you. Scenario for you. Had to lower back injury. Going to physio for a year they could only do so much. Going to a chiropractor you can only do so much. Last week horrible pain in the the glutes feels like piriformis syndrome. Sensation goes from the back of the leg to the glute Maximus possibly the Medias then to the lower back. When I find is when I press into the S1 facet joint when sitting it relieves a sensation down my leg. Causing more mobility now in my lower back. what do you think I should do to fix it do you think I should get an injection?

Not fair for me to comment without looking at you, the L5/S1 facet can refer pain - however, could be a multitude of things so having the injection might help, as long as it is involved with your symptoms, regards JG
