He's basically relapsed and SH'ed. It's referred to in the comics too, just Charlie's POV

I know someone else answered but I love solitaire; her younger brother Charlie has diagnosed OCD and has a SH relapse while frantically reorganising everything in the kitchen while Tori thinks Charlie is hanging out with Nick (Charlie’s boyfriend) but it turns out Nick stormed off (he’s not a bad person just 16) after a fight they had and left Charlie alone, stressed and scared. One of the stressors in this chapter is that Tori blames Nick for leaving Charlie on his own while he’s in distress when he explicitly knows what happens to Charlie when this happens but nobody is to blame here, Nick had the right to leave as he was distressed, Charlie couldn’t help his conditions flaring up when he was distressed and Tori couldn’t help being protective and upset on her brothers half especially when one of the focal points or Tori’s character is that she won’t accept her own mental illnesses because she thinks it would take away any of the seriousness of Charlie’s suffering and that she doesn’t feel like she completely relates to Charlie - as she wouldn’t, because mental illness and it’s expressions are a spectrum or completely different diagnoses’. In this case, Tori struggles with OCD-like tendencies and overbears her brother in order to perhaps treat herself or suppress any OCD-like behaviour she has alongside her depression. I really relate to her character as my older sister has a cluster personality disorder, and when I was younger I’d suppress most of my negative feelings in order to not be her. TTYL; I like this scene.
