Crazy how many people just assume Ito Manabu character is a trans. Ito even stated that he's not nor does he have a multiple personality disorder to look himself as a woman. He's a cross dresser , that's it. I'm glad that you mention how Nakoshi sleeps cuz I have my own theory on that. (SPOILERS if you continue to read this) Is that while one part of his mind want's him to go back to begining as it all started (being an infant) .His other part subonciously filling him with guilt of his abandoned baby. Why I think that way ? It's cuz of Nakoshi emotions when he found out that Mr. Ita (Homeless Guy) also abandoned his daughter . Nakoshi looked so heart broken at that moment but not so much when Mr.Ita was gone.

you’re so wrong & purposefully leaving out information… Ito took a long time to accept this about himself (which is why he denies it at first) & even later on acknowledges that he will likely get gender affirming surgery. nothing wrong with being a cross dresser but they do not get that type of surgery. Nakoshi even later sees manabu’s homunculus as breasts. It’s a very clear and obvious depiction of a trans woman who is learning about herself and exploring her identity. Nakoshi even says “some people lie to themselves by changing and others lie to themselves by not changing.”

I read this manga long time ago, I think is really weird, you cant compare with a traditional Shonen manga, if you want to make comparisons you need go to Seinen an in that go to the psicological mangas, like Oyasumi pun pun, or maeby others because is a bizarre story of a man who after the operation start to help others to fx their traumas and then carry them in himself, to the point that in the end he can't even understand but ends acepting himself like he is, to the point of that really weird end were I can only say he is totally broke but happy.
