It's a really weird way to end a joke or monologue and if it's not spoken the right way, can sound really dark and disturbing. I think of the person speaking as a character who kind of knows what's supposed to make a good stand up joke.. but not quite. So when he says the last line, he's looking for a funny/different way to say "and they were staring at me." But instead he completely botches it and says something horrific. A live audience would gasp. Michael Scott doing standup comes to mind.
