70% is the minimum eligibility for applying I think but usually students getting admission in LUMS have higher percentages.

After logging in, form option should be available there if the NOP application is open. Try logging in using a laptop or computer, sometimes logging in using mobile phone causes confusion.

Yes you can apply for both but in case of NOP you would need to wait one more year because the NOP process takes almost one year including the NOP summer coaching session.

W.salam… you have to share the details of these activities in your personal statement and it is recommended that you provide the proofs if possible, and if you don’t have formal proofs like certificates then you may upload any relevant real pictures of these activities. However, if this is also not possible then at least add the details in your personal statement without uploading proofs.
