In this video I explain the criteria for brain death and how it’s different from a persistent vegetative state. Let me know if you want to know anything else about brain death. 🧠

I have a question why does it take 2 days to make determine brain death Someone I know is on a ventilator and that’s the only thing keeping him alive is the machine ..the doc told me that the only part of his brain that works is him dreaming 💭 is that consider brain dead or does the brain have to stop working period to be considered brain dead ..

My dad who's homeless and a heavy drug user..collapsed on a porch. He is thought to have been without oxygen to his brain for anywhere between 5-20 minutes, they think closer to 20 minutes. Coded twice on way to hospital. His eeg came back as being a 3 out 15. He's on the vent and they have him on a sedative. I'm going to have to make the decision on whether to take off vent or keep on the vent based on other family members are trying to push me to do. It will be 5 days this Friday. My question is he has done involuntary movements like coughing and moves foot when reflex is performed does that mean he's totally brain dead or his brain stem is working just not the rest of his brain or is the vent more then likely keeping organs alive?

2 years ago, my sister was declared brain dead. When found blue, we don't even know why. At least that's what her shady husband said. Hit her phone from me in the wall but gave me all of her belongings and her when he cremated her and would not Bury her. Cause it was too expensive. My brother being younger than me. What's holding it better together up in the hospital room? And it's not about drama so whenever he wouldn't continue telling us me and my younger brother. Why my Baby sister of 30 years old was like that. I thought that was shady and started asking him to tell them. He looked at them a certain way as if he briefed them about something. I walked out they followed me out and now today my brother declares that they remove me from the hospital. I know God damn well they didn't. They can pull up my CCTV from my sister's room. They did not ask me to leave. They didn't look at me weird, they didn't grab me. I walked out of my own recognition. They followed me out to the g****** parking lot. When I was on the phone with the detectives? I had it with him. By then he came outside took me and my brother home. After picking itself to take us over there to say Goodbye the day before. She bought my dog a harness in a leash. She always did things for me. She was wonderful mother of 3. She had to leave her baby's father. That was raising the kids with her because he was cheating on her with a boy. So she married a man that was giving her things that she didn't need. That man wouldn't tell the actual reason why my sister went into a brain death state. It is almost 2 years later and I can't fight alone. My brother Is the only one that was with me in that hospital room and in the ride to the hospital with homeboy. Talking about what he had to tell his daughter about my sister. The night before this happened, he didn't continue and I tried to get it out of him. He was scared to continue because he knew he was going down or losing his daughter and his wife. He wouldn't just lose his wife. His daughter's going down for killing my sister. My sister did nothing but b**** about. His daughter would not leave selling drugs out of their house and she did her drugs and it was fake drugs because her new husband gave her the drugs from his daughter or bought them from his daughter. His daughter thought he was killing 2 birds with one Stone by keeping the money in the house, taking it from the girl that was taken the money from Her daddy He owned his own home and would not make his daughter leave. They raised kids in the same house they sold drugs out of. I am going to do something about it, but it's not on his time. The government should have already asked for her phone and his and would have found out. He was enabling her to be drug-free. When she was fighting to get her kids back because of accusations. Accusations that I know were true now. He was helping her put the loose around her neck. And he acted like he loved her up until. She had to be cremated because it was too expensive when she was in life support. I went home list when my Grandpa died. My sister was in rehab. I put all of my things in my storage unit. My sister died. He gave me all of my sisters. Things that did not even help me. Keep my storage unit. And that was the only thing that we worked for was to keep that storage unit until we got a house. Because that was all of my family's belongings and hers. He didn't help me keep he needs to go. That is the story of jennifer In a nutshell. God pray for me

I know two months later means the odds aren't so great, but I'm wishing you and your friend the best. One of my best friends recently lost his little sister after two weeks in a coma, and his family, which are now staying with me, are more than a bit distraught, and I'm trying my best to use my medical background to assure them that she was given the best treatment she could've been offered, but the communication process for families going through this sort of process really leaves much to be desired. In particular, unrealistic expectations where there should be none...

I lost my only son at just 18 to a dui driver, he was pronounced brain dead eventually after 5 days. As a "common man" lol, a few suggestions I'd give for patients & Dr's is communication is paramount. The Dr. did a good job, but after all machines were disconnected with a fully collapsed R lung he began to sit up his arms crossing which is actually common but to not know this would happen made it much, much more traumatic. A hopeless father like me is still clinging on to any shred of insanity or hope because the other option that he's gone is just too painful to consider. They told us he wouldn't move in fact that he couldn't even reflexes he would literally just stop breathing in 20 seconds & that would be it quick, quiet & done no struggling. There's the Lazarus effect & common to brain injured patients to have their body respond almost like a pause button at the point of impact right before death so the body can flinch, tense up, etc. Only other thing is be mindful of the organ donation corporations. Nothings worse then having them come in 2 sometimes 3 times a day with papers to sign I get the whole process I'm a organ donar myself but people are in shock & grieving give them time if possible

We donated ours son's organs. I found lifenet to be very respectful and we requested for his organs to be donated after his passing was confirmed. Yes the process is alot and it sucks to deal with so much in that time. Our experience was very respectful and easy considering the circumstances. But then after death you have to deal with funeral home arrangements so that is a whole other necessary but unfun experience. Sorry for your loss.

What about someone who accidental over dose on fentanyl but not a fentanyl user , and MIR comes back with severe brain damage , what are chances in survival ?
@TheIntenseMD
@TheIntenseMD 3 жыл бұрын
Great question! Brain damage is a complex topic. We rely on imaging, neuro exam, and the neurologist (brain expert) to help us with giving a prognosis in each case. Something important to note is even if imaging doesn't indicate brain death - there still could be a severe degree of brain injury. In these cases, a patient would require supportive measures such as the ventilator and feeding tubes to survive, so it comes down to a question of what the patient's loved ones believe they would want in this situation and what quality of life is acceptable to them. Some people do not want to be kept alive if they are not able to interact with their environment/loved ones and are maintained by machines/artificial support. Sorry for the long winded response, but this is a complicated topic. I hope you found it helpful!
@eYeZiQ
@eYeZiQ 11 ай бұрын
@@TheIntenseMDLong-winded of that sort is always appreciated. One of my best friends recently lost his little sister after two weeks in a coma due to fentanyl or xylazine [10 minutes unconscious prior to paramedic arrival], and his family, which are now staying with me, are more than a bit distraught, and I'm trying my best to use my medical background to assure them that she was given the best treatment she could've been offered, but the communication process for families going through this sort of process really leaves much to be desired. In particular, unrealistic expectations where there should be none. A short period of breathing on her own without a ventilator doesn't indicate much of anything, but I don't know how to communicate that to them with out upsetting them. The brain stem is a powerful machine that operates in fits and starts with / without conscious intervention, if you ask me.

This is very good information. I have a question for you. I read where if any part of the brain stem is destroyed that you are brain dead. I know that the medulla oblongata controls vital functions but is it really true that the destruction of the midbrain or the pons can cause death?
@TheIntenseMD
@TheIntenseMD 3 жыл бұрын
The majority of the brainstem would have to be damaged for the patient to qualify for brain death. In this situation, the patient would not exhibit any of the vital functions we test for. It is possible for a patient to have a pontine or midbrain stroke and not have signs of brain death. As you stated, the medulla oblongata is the most critical portion of the brainstem. I hope this was helpful! Thanks for watching!

My brother n law went into the hospital on Friday morning. He suffered some type of cardiac arrest that resulted to him passing out. He had brain swelling and was not breathing for a length of time before the paramedics came and revived him. He was rushed to the ER. He was experiencing these jerks/spasms every other minute. During these times his blood pressure would shoot to the roof and then go back down. They placed him on propanyl to reduce pressure and stop these jerks/spasms. They took him off the propanyl 24 hours before declaring him brain dead. Several people performed neurology exams on him/ water in ears pupil test, corneal reflex exams, ventilator stopping, EEG, im just not sure if they checked his levels to make sure that he had no propanyl in his system and a couple more other things you mentioned. Could the propanyl have caused him to fail these neurology tests? On the EEG machine the lines would spike up when he had these spasms but by them putting him on Propanyl they stopped. Was that brain activity? Could he have come back if they did not give him propanyl?
@Raison_d-etre
@Raison_d-etre 2 жыл бұрын
No. Obviously, the part of the brain that controlled heart beats was damaged, and therefore bp was not controlled. Propranolol is a very benign medication. It will not stop the heart, but it will calm it, and it did in your brother's case. It may have subtle effects on the brain but cannot do anything close to affecting neurological exams. There isn't a documented case of DOCTORS declaring a patient brain dead and subsequently proven wrong. The stories you may have heard do not involve DOCTORS.

Hello Doctor, I hope you can see this. I have a brother that's is currently on the hospital that has diagnosed with catastrophic Brain damage due to his heart stopping for between 10 and 15 minutes. They mentioned not getting oxygen to the brain and damaging it that way. There are many things that I don't understand because some doctors explain things a little different than the others. The tricky part is they thing that he is breathing on his own. They say that they chances of him getting better are not there because he didn't respond well to all the stem tests. How can a person be brain dead and still breath on his own?
@han_abby1
@han_abby1 8 ай бұрын
Keep talking to your brother and aask for and look for responses. It could take many months to see small improvements, but don't give up on him. What if they have no brain activity but still has a hearbeat and is on the ventilator breathing over 30?
@TheIntenseMD
@TheIntenseMD Жыл бұрын
Great question. Breathing over the ventilator requires brain stem activity. So they do have brain activity but possibly very minimal. Apnea testing is part of brain death testing. Apnea means the patient is not producing breaths (absent of breath). If they do trigger breaths, then the patient is NOT brain dead, however they may still have severe brain injury if other functions are lost. I hope this answers your question. Thanks for watching!
