Anal fissures are small, usually longitudinal tears within the mucosal tissue of the anal canal that typically extend from the pectinate line down towards the anal verge.
Infected anal fissures can lead to abscesses and fistulas. An abscess is a localized pocket of pus caused by a bacterial infection, and causes pain, swelling, and tenderness (these symptoms go away when the abscess is drained). Like other infections, abscesses can cause chills, fevers, and fatigue. A fistula is a small channel, almost a tissue burrowing, that develops secondary to an infection. An anal fistula can move through the sphincters and exit via the anal skin, but it can also exit into the vagina or bladder.

→ Topical nitrates: 50% healing rate, 20-30% chance of severe headaches, 50% reoccurrence rate

→ Botox injections: 50-80% healing rate (evidence suggests it is only slightly superior to topical nitrates), 0-15% risk of fecal incontinence, 35-50% reoccurrence rate
→ Lateral internal sphincterotomy: 90-100% healing rate, 0-30% risk of fecal incontinence, 5-10% reoccurrence rate

Hi I have this Annal fissure since Feb 2022.I try everything and ointment cream sitzbath and I changed my diet nothing works and the pain it's awful.I went to the hospital and they same me home because they already gave me everything for these .I can't take it anymore 😢

I'm sick of having rectal exams! I've been dealing with this for over 2 years and the last 3 months I've been in daily excruciating pain for hours - up to 12 a day. My life has been horrible lately. I am hoping with all my soul that this video can help me.

​@@aliyamajeed3889 I was/am not constipated. Usually the other way (diarrhea). Unfortunately not too much has changed for me yet. I'm going to be getting Botox injections this week to see if that will allow for things to finally heal.

Fantastic video- you answered so many of my questions! Thank you! (I am about to have rectocele surgery.) My surgeon is arrogant and not very good at listening- prescribed topical nitrates despite me telling him I get daily migraines- so I used that product once and only once. I was using a product called Aquafor as a barrier (its like vaseline but not as sticky) in lieu of powder to prevent itching. I have never really been a fan of powder because I live in the American SouthWest-my skin is already dry enough! -and I had a bad experience involving leather pants, lotion and baby powder (together they make a paste! This, in turn, causes great strife when an overheated human female is trying to remove said pants. And you can just forget about putting them back on... which isn't so bad, unless you happen to be trapped in a public toilet.) ANYWAY, You are the only person to actually make a case that swayed me to try it again because I do live near the surface of the sun and have a little extra junk in my trunk, I tend to sweat more I have to say, I am so grateful to have found your channel- I think it was fate because I found you before I was diagnosed with pelvic dysfunction. Have you done any videos on how to exercise -do's and don'ts concerning pelvic injury or pelvic floor dysfunction. I recently discovered I WILL NOT be riding a bicycle (unless recumbent!) any time soon due to a perineum injury, so I am just trying to figure out what I can do at home to recover. I also wonder about sitting/walking too much with the injury. I am an artist, so I sit a lot for work.

I'm so glad that this video was helpful for you! I think I remember when you found my channel, was it to do with my GERD/gut health & birth control videos? I think I remember you! I'm so sorry to hear that you're navigating an upcoming rectocele corrective surgery and a perineum injury, and that your surgeon isn't very good at listening. Thank goodness you didn't stay on the topical nitrates considering your history of migraines. Has your surgeon or have you considered pelvic physical therapy after surgery? Sometimes getting a 6-8 week post-op (pending on the surgeon) appointment can be super helpful. A pelvic PT can help you rehab your pelvic floor, walk you through safe exercises (and help you with your questions about sitting/walking), and teach you how to best protect the longevity of the outcome of your surgery! Yes! I'm familiar with Aquafor! Oh my gosh, your story about heat, leather, and baby powder sounds so unfortunate, I am so sorry that happened to you! I don't blame you for choosing Aquafor over baby powder considering that experience! I don't have a video about safe exercises for pelvic floor dysfunction, but I love that idea! I've always struggled with exercise content because I get in my head about the audience not necessarily knowing if the exercises are safe/appropriate for them and then getting hurt/making themselves worse, but I also understand that some of my viewers don't have access to pelvic health services and are doing their best on their own. I've gotten a number of exercise-related requests now, maybe I'll do my best to start creating that sort of content but with a thorough outline of who should/shouldn't be partaking in the exercises first!

Wow you really did break down and explain better than anyone on internet or doctor or surgeon I have consulted I’ve now suffered for over as year and has destroyed my wonderful life previous to this horrible pain of a fissure My big mistake was not being patient tried the GTN then had Botox and 6 weeks ago had LIS Surgeon said after 3 week was about 75 percent healed but when he checked today Looks like I set I back to 50 percent a moment Having 6 hours pain after B M on hindsight If I had use the GNT and followed your instructions I believe would been better off Today you are definitely the best on line by miles so I hope people listen to you This problem also effects peoples mental health due to the location

@@bridgetjones5695 Thanks for asking. I am doing pretty well. My family doctor prescrbied me nifedipine cream + stool softener. I felt relieved during bowl movement right after I start using the nifedipine but I came across another issue which was a nasty pain that could last up to 8 hours after bowl movement. it was more painful while I was driving. Two weeks later, I start feeling better, and pain after bowl movement became less painful till the point that I am kind of fully recovered in terms of pain but still the fissure does exist. a skin tag right next to the fissure what I believe is the source of the pain after bowl movement. I have an appt with proctoligst soon to confirm.

I remember I had nausea and headache for few hours in first apply. I beleive felt that way coz I read about it before I start using it. Its better not to think about it and get busy with your daily activites. I use laxative stool softener. It comes as powder so I mix 1-2 tbs with water everyday in the morning.

I found that my constipation was caused by gluten and dairy. You might try eliminating these things and see if it helps. Good luck

Excelente Video, thank you for all the explanations! I ve tried nitroglicerine for some Days and stopped as i feel still the itching and very bad headaches. My fissure got completly healed and came back after a colonoscopy exam. For me the itching is what is causing a lor of skin inflamation. It is a very painfull situation, it can be very challenging mentally and emotionally, i ve also treated with different kind of therapies Strenght, compassion towards myself and self love i ve learned a lot ❤

oh my goodness. I had no idea sitting on a donut could be the cause. I had a cyst removed and sat on a donut, even driving with it for over a year since it became comfortable. No wonder I can't get this thing to heal. Thank you for this informative video!

I love your videos. They are very helpful. Suggestion...Under preventative...I was expecting to hear you weigh in about food choices & fiber drink options (psyllium husk, guar gum, etc.) If you discuss it in another link, please let me know. My understanding is 35g of fiber daily is the goal. I'm finding it really difficult to come close to that #...even while eating a generous amount of fruits & veggies every day.

Hi Tori, love your videos. I've had gastro/stomach issues for many years. Long story short, seen 2 gastro docs and one surgeon. I've had much testing over the years. Nothing has solved my problems, it's probably getting worse. I just started physical therapy for pelvic floor dysfunction and I'm hopeful, but it's only 6 total one hour sessions. How do I find someone like you to help me on a regular basis, which I think I need at least for awhile. Thank you!

Thanks for this great informational video - very well explained! Will you be able to provide some treatment options for sentinel tags (that grow around fissure) to reduce their swelling and pain? And is there some way to shrink//remove them?

I did respond to your comment on my other video as well, but I wanted to say that I definitely think some of the same concepts for healing fissures/hemorrhoids would help decrease tags' swelling and pain! Namely not hanging out on the toilet for longer than you need to (the tourniquet effect would be provocative I think), normalizing stool consistency, and avoiding straining activities!

I'm glad this was helpful for you! Reoccurrence can happen with diarrhea as well as constipation, unfortunately. It can also happen after having to strain more than usual with a bowel movement (even if it's not a constipated BM), or after having a larger BM 😔

The efficacy you stated for the various treatments, are they all coming form the same state? In other words, is Botox x% effective at treating any fissure or only the ones that have already tried, for example, months or years of conventional treatment?

I had two rounds of botox injections for mine but they didn't help! what helped was keeping stools very soft and the calcium channel blocker was a life saver. the nitrate creams sent me to ER with the blinding headaches. if I eat badly for a while my fissure will open up again but can be healed within a month or so. My surgeon refused to do a LS as I'm 24 and the risk of incontinence was too high

First of all, I am SO sorry to learn about your experience with nitrate creams! I cannot imagine how scary those headaches must've been if they sent you to the ER, but I am so happy that you figured out how to normalize your stool consistency and that that ultimately worked and continues to work for you. So interesting about the risk of FI with the LS! Of course statistics and real-life implications are different but I'm really glad that you opted for the calcium channel blockers since your surgeon was worried about the risk. I'm definitely weary of the risk with my FI experience in the clinic. Secondly, thank you so much for sharing your experience so openly here (and on other videos!). I appreciate it and I hope your comment helps someone else either in their healing, in being reminded that they aren't alone, or both 🧡

hi...i love your videos. my biggest problem is i can never tell which day is constipation and which day is diarrhea. it could be both in one day. its never ever consistent. iv been to doctors for 4 years. with no solution do you have any tips for that?

I'm so sorry that you're struggling with inconsistent constipation and diarrhea, and that the doctors haven't been helpful. I'm honored that you're comfortable asking me for guidance; however, I cannot give individualized medical advice. I can only share general information. That being said, I have some general information to share 🤗 A few things to consider... if it's a mix of constipation and diarrhea, it makes me wonder about digestion (I'm the most curious about stomach health, small intestine & large intestine health). I'm curious about SIBO, I'm curious about mixed IBS. I'm curious about medications (if you started taking something around the same time these symptoms started, including birth control). SIBO, mixed IBS, and pharmaceuticals can all interfere with digestion and cause bizarre, inconsistent BMs. Have you considered those ideas/ruled them out? And thank you so much for the kind words about my videos, that means a lot ❤️

What is a natural stool softener is water, but the it needs to be room temperature, so cold water, cold drinks and cold foods are the culprit, it causes constipation and hard stools.... I noticed that when I switch to room temperature , hot foods and drinks , my constipation went away along with the hard stools, I know it may seem refreshing for cold drinks and cold foods but it really messes up digestion, I could not believe it myself, maybe for some it does not affect them, but with me it sure does... So with me, I do not drink or eat anything cold anymore, yes it can be in the refrigerador but I will heat it up in the microwave.....or I will drink very little milk or juice if it is cold..... this has changed my life..... even beer, i drink it at room temperature......no ice or ice cream....cold soda and excessive sugar foods /drinks. Guys and Gays, give yourself a week on this and say yourselves if there is a notice of difference , with me there was a complete difference

I have a lower back problem, so I got myself a sitting donut for my chair recently and now I have an anal fissure problem. You mentioned about the sitting donuts are not that great in the video ! Now, I stopped using it. I wish I knew that before... In my case, does the anal fissure heal if I treat it the best possible way at home?

Been suffering with anal fissure for a very long time due to it being originally diagnosed as hemeroids. Only recently been told it’s a fissure. Tried nitrate gave me migraines, then tried calcium.. helped but didn’t heal. Just yesterday had botox and skin tags removed. Feel a lot better but like you said can’t control gas! Still a little pain but nowhere near to what it was a day ago. Came across your video, amazing how you explained it! I’m sure you are a great doctor just as you are a teacher. Hopefully this Botox treatment works, also if you do see this message please advise if i am still able to use my diltiazem after having Botox injection? @@barshabhandari281 honestly it didn’t help at all, if anything made it worse. Since then I came across Pranicura, it’s been a god send.. pain is gone and feeling so much better. They also refund you if it doesn’t work for you but you must follow their treatment as outlined on their instructions. Good luck, hope you get better !!

I'm not sure if I have a fissure or hemorrhoid, the hospital said they thought it was a fissure but I did not have an exam, but it stings to use the calcium channel blocker and I'm afraid to stick anything inside because of the pain. I'm taking a mixture of Tylenol and ibuprofen while I wait months to see a surgeon for a diagnosis and I'm scared these will cause serious damage if I'm taking them for so long. Will the calcium blocker still work if I only put it on the outside? Thanks for your videos they are peaceful and easy to understand for someone in pain.

I'm so sorry to learn that the hospital didn't do an exam/give you a proper diagnosis and that you have to wait months to see a surgeon! It really upsets me that the hospital discharged you without a proper rec*al exam. If you're able to see your primary care physician or even if someone you love can take a look and just rule in/out anything obvious (something of substance, swollen and hanging vs. a tear) - that might be helpful just so you can have some direction in your treatments until you can see your surgeon. Know that I cannot give individualized medical advice, I can only share general information/resources. That being said, the application of the calcium blocker and its efficacy depends on what you're trying to treat and where it's located, so it's really hard for me to give you any resources/information in response to your question 😔 Prolonged use of any drug comes with potential side-effects. While ibuprofen and Tylenol are relatively safe drugs, if you're at all concerned, ask the provider who encouraged you to start taking them together (they're very effective for pain management when taken together, and are sometimes taken together for longer periods of time, a couple weeks in a row maybe, but there are other pain-relieving options available if you're concerned!). I'm really sorry that I can't give you clearer answers or be more helpful, but I'm so happy that you enjoy my content! There may be a physical therapist in your area that has been trained in pelvic floor problems. Ask around (any PT should be able to guide you to someone who does this). Also a good book that can help is “A Headache in the Pelvis”

Apart from the home treatment, i'm using GTN in the morning and at night, even though I get headaches. Im aldo using Lidocaine. I dont know if im doing right, because the burning pain still seems to be there. Ive not decided to fast for maybe 2 days, taking Laxido (stool softener):in the meantime. Really fed up with it, as I have problems with my mood anyway😏

Doc, why is it recurrent? It healed for a month with topical Nitroglycerine and then it came back! I would like to avoid surgery and I'm thinking it's my diet which needs to be overhauled. I've been eating brown or wholewheat carbs as my staple but I suspect it got triggered with garlic, beef and sour foods. Hoping to hear from you! Thank you.

While I'm so glad that you were able to heal, I'm so sorry that the fissure has returned! Often times fissures that return/become recurrent won't stop recurring until you treat the true "underlying" issue - normalizing stool consistency (constipation/diarrhea) tends to be the most common/is a great place to start, and diet, like you're suspecting, can absolutely play an integral role in both irritating and healing fissures. Unfortunately, I cannot give individualized medical advice, so I cannot guide you in how to specifically change your diet - but it sounds like you're on an intuitive healing path! I'm sorry that I can't be more helpful, and I hope that you pinpoint the "underlying" cause, remedy it, and find your healing soon 🧡
