Yes, tossing roughly 4 inches beyond the tip of the racquet head makes sense. From that toss apex, it will drop about 8 inches before contact. Federer's first serve toss appears to drop about 12 to 15 inches before contact. Have been experimenting with a 12 inch drop from the apex to contact , and I do get slightly more power when this higher toss is accurate but my toss accuracy suffers -- toss tends to drift around. I find that the 8 inch drop from the apex to contact is far more accurate for me.

Yes the height of the ball toss is fairly individual. The height depends upon your style (some styles take longer for the racket to get up to contact) and a few other factors. What we know is that it needs to be above ideal contact point but not so high that it drops more than a foot. I don't think that recreational players can base many things on what the pros do. The ball toss is one of them. Many top pros have different forms that they have developed and practiced over the years that none of us should emulate. It's best to go for clean lines and simplicity on the serve.
