I remember my ultra hard I had gotten guppys paw and pre nerfed soul converter on the same floor. There was some other crazy stuff too, either poly or proptosis (can't remember) but I remember doing boss rush and hush just for the hell of it. I nearly went to delerium but I was worried it would not give me the trophy. Good memory.

I remember my ultra hard run was pre repentance and I literally just got carried by D4 from my first treasure room. (Back then D4 rerolls would apply health from newly acquired health items directly to your health bar) making it an efficient way to pretty much fully heal with every use. Only downside was random items and the risk of breakfasting.
