Thank you for this! Quick question - if we had paid cash for some equipment and lease other - would you put any cash paid items in equipment - but put the leased equipment in fixed cost? Also how do you account for utilization? Meaning if you have someone on salary @ $200 per day - whether he runs one thing or 20 things - it is the same cost. Thank you!

I put all the equipment in there regardless of it being a lease or cash. It helps account for the future replacement. You can either add the salary as a fixed cost or you can just assign the FTE to one of the machines. It just depends on how you want to spread the costs around. Either would work.
