How I Matched into a Fellowship in the USA as an IMG ?

Does securing a fellowship position require a high score in step 2 which means it requires the same scores as the ones you should have to apply for residency or you just need your ECFMG certificate?

Pediatrics is one of the less competitive specialties. Getting in would depend on how strong an application you put forth, opportunities, your CV etc. You can carefully approach this with a strong application www.IMGSecrets.com

Doing residency again you mean doing the 4 years of anesthesiology (1 internal + 3 anesthesiology) or only the 3 yeats of anesthesiology?

Directly becoming a consultant is an option and that would depend on how strong a candidate you are. Very few candidates are eligible for this path to by pass residency. www.IMGSecrets.com
