My aunt used to work for Motorola before the chip fab was spun off into a seperate company. Supposedly, there was a few weeks where every platter of chips ended up getting mostly rejects and they couldn't figure out why. Motorola did everything they could until someone was on the receiving dock and noticed the new guy rip open a pallet of hermetically sealed platters and counted them by hand. Once they fired him the acceptance rate went back up.
