I think you can use the label text directly, enter $length there instead of selecting an attribute field

how to choose between cartesian and ellipsoidal length?

Cartesian distance is as measured on a flat Earth. Ellipsoidal distance is as measured on a spherical shape (or ellipsoid). so if you are measuring short distances it doesn't mater too much but if they are longer distances then you should use the Ellipsoidal one since it is more close to reality
