When should we remove the headrest part? Or should that stay ?

You can continue to use the head insert if you prefer. The head insert must be fastened in the slot above where the shoulder straps. It cannot be fastened in the same slot as the harness straps. Once the harness straps are in the upper most slot you must discontinue using the head insert.

When do the straps need to be adjusted? What is the recommended weight/height?

The straps should be at or sightly below the baby's shoulders. There is no height or weight. You need to monitor as the baby grows.
