Thanks for your teaching in sensitivity and specificity My question... In a group of 50 people with corona disease and another group of people without corona disease and blood taking from them to determine the serum of both group. 40 out of the people that corona tested positive and 6 out of the people without corona disease tested positive calculate sensitive and specificity

to make sure i am under standing if we have a 100% sensitive test this means that all diseased we be included in the positive tested group and all the tested negatives are really no diseased , but on the other hand if we have 60% specficity for this test that mean many undiseased will be included in the tested positive group?? hope i am right

Hi, How do you calculate the 88% sensitivity and the 90% specificity ? I don't understand if you have to do 220/220+30 or something else.

Hi Milou, Sensitivy = TP / (TP+FN) = 220/(220+30) = 220/250 = 0,88 (Multiply by 100 to get a percentage) Specificity = TN / (TN+FP) = 675/(675+75) = 675/750 = 0,9 (Multiply by 100 to get a percentage) So your calculation was correct. Clear?

Do you mean confidence intervals (95% CI)? They indicate the precision of results. Confounding is rather a problem in cohort studies looking to find a causative relation between a factor and the outcome (development of the disease p.e.). Think you mean bias.

Is it possible to calculate PPV, NPV, Sensitivity and Specificity of each test Separately with out gold standard ? Could you guide me please. No it's not possible then as you have to have some kind of proof (through the gold standard) that people are sick/healthy. What you could do however is to calculate the reliability of the two tests.

Yes, of course. We can theoretically have 100% sensitivity and 100% specificity at the same time in a completely perfect test. There is however usually a trade-off between sensitivity and specificity, meaning that researchers usually decide for a cut-off point with the so-called area under the curve (AUC) for which they obtain a desired sens or specs value.

if i want to screen patients for UTI preoperatively to postpone those with UTI, HERE i will use a highly sensitive test to detect most of those with `UTI and postpone the operation.But some of them may not truely have UTI and will be postponed in vain. While if i use a highly specific test i will be sure that the tested positive is really having UTI and will be postponed, but some of the tested negative will have UTI and will go for operation !!!! Right?

UTI = Urinary tract infection? Screening: highly sensitive test to rule out in case of a negative result. Positive result can be false positive. Confirmation: Highly specific test to confirm in case of a positive result.
