Always go through both loops on base row. You wont have a loose hole then.

I dont know what im doing wrong 😭 I always end up with 1 stitch left over in row 1 and ive made sure i followed to a T

Hi! Are you sure you count the stitches right? Remember that we don't count the loop on our hook. Another thing to keep in mind - the basis of our foundation is the multiple of 4, but then we add 3 more stitches to create a turning chain. Take a look at the diagram (on my website - the link is in the description box), it may help you visualize the first rows!
