Have you noticed the proximity effect at work in your mix? How do you typically address it with your singers?

Proximity effect is the best and sweetest part of the microphone. Microphone without Proximity effect is not for me) I will never spend money on the microphone that doesn't have that effect. Clarity and the contract of the microphone signal is the key factor.

I'm a singer and I have a great question for you guys. From 3:43. If you notice guys like Frank Sinatra and Dean Martin live, they are like a foot away from from the mic (even with an SM58).. lol.. How do you explain this? How does their voice still sound so loud? With an SM58 if I sing more than an inch or two away, there is barely any volume. The signal is pretty much gone as discussed around 3:43.

A Metalhead's Journey probably need a condenser mic with phantom power a lot louder than cardioid dynamic ones

As T mentioned, probably a condensor, but also Frank Sinatra actually keeps the mic the same distance from his mouth pretty consistantly.
