Annexure A need to be printed on a letter pad (generally with office name address and logo on letter head). If your office doesn't have specific letter pad you can go with plane paper with your office name address typed on letter head. Annexure G and H don't need any letter pad just take a print out of format and fill it you are ready to proceed.

Attest it from deo/beo. You need to submit it at passport seva Kendra with other documents on the appointment day.

It's better to provide noc as there will be no police verification if you provide noc.

Completely incorrect information. Don't make videos if you do not have proper knowledge. In Annexture A police verication is not needed. Neither before nor after.

@@TravelHelp Just yesterday I got my passport. I read your vlog 2 months ago when I was preparing to apply online. First I got Annexure A from my Dept for self and two dependants and then I applied online. When I went to passport office they only asked for IC ie Annexure A for all three individually. Not NOC or PIL. They told me police verification will not be done. Within 3 days of documents verification I received my three passports by speed post. So it's not theory it is a fact and everything that is written in passport website is all true word by word. Make changes to your video so that people like me are not misled. Are you a govt employee and did you get Annexure A from your office? Or did you got PIL instead?

Where you from? Kudos to your state . But in my case passport office intimated police station for clearance even I have applied with annexure A.

@@TravelHelp States should not matter. As passport offices are all central govt anyway. Same rules for all should apply. BTW I am from West Bengal. And not only me many people I know from my organization got in the same process. The matter should be clear when you visited the passport office. I think they did not accept your IC document at that time for some reason. Generally IC are issued from Head Office which are mainly in New Delhi for Central Govt Depts.
