Thanks for a very informative video. I have question on Personal History and Addresses section of the form. A bit of context: My work involves a lot of travel outside of my country of residence (Dubai, UAE) to lets say Qatar/KSA etc. and hence I keep staying in different addresses for short duration (a week or two weeks) and then I get back to my country of residence (Dubai, UAE) or even sometimes to my country of citizenship (which is not UAE). So should I mention each time I stay in the same address since from/to time keeps changing (in case of weekly stays)? And then should I repeat the same for Personal History section as well (my employer is same though) or should I keep Personal History section at the level of employer only (which is UAE based)? Appreciate your prompt support due to time sensitivity of the ask.

Why do u think primary education would be grade 1 to 8?. I search on the internet and it mentions that grades 1 to 7 primary and std. 8 to 12 secondary. I have added the name of the apartment along with the street name. Will that suffice if not how do I create the extra space in the form? 2. For the education section I got a little confused. I have done my education from class 1 - 10. class 11 - 12 as college, bachelors for four years and master for three years. You can create a word doc, create the same table and provide full detail. Then merge the Form and the word doc as a single pdf and upload it. This is in regard to your first question. Same scenario as mine. I hap provided all the details in the video itself(3:46 to be precise). Thanks!!

Hi Sandeep Thanks for sharing the video. I and my spouse have been struggling with form no. 5669. It seems like the instructions provided on the checklist form no. 5533 and the latest version of form no. 5669 (06-2019) does not match each other. As per the checklist 5533 this form should be validated and signed by the following individuals’ principal applicant and the family members. However the form has no radio button for Validated and for the family members to sign. Sorry about the wordy message but would appreciate if someone can help.

You need to fill this Form for yourself, your spouse and dependent child aged 18 years or older. So for example if you don't have any kid yet, you need to fill two copies (each one for your spouse and yourself) and sign it. I don't think there is a need to validate this Form as nothing has been provided for the same. Not sure if I understood your question correctly. Let me know if this helps. Thanks!!

Yes Ankit, you need to provide residence details of last 10 years. E.g. if you are married since 2015, you need to provide before marriage address of your spouse from 2010-15. Hope this helps!!

Aman, these are tricky questions and would suggest to gather as much information as possible through different mediums. I would suggest to fill both the fields (First name and Family name) and also attach an LOE (Letter of Explanation) that how it is present in your Passport. This is the best I can think off. No need to attach refusal letter if they haven't specifically asked for it. you can merge two PDF files and then upload it as a single PDF. There are a lot of free websites (e.g. SmallPdf.com) with the help of which you can do this. Thanks!!
