The nasal septum refers to a central cartilage that separates the left and the right side of the nose. It's really important because it directs airflow from the front of the nose to the back of the throat and down into the lungs.
Any time you have a bowing or deviation of the septum to one side or the other, or even both sides, you're obstructing that air flow. This is what we call a deviated septum, or a nasal septal deviation.
The most common symptom of a deviated nasal septum is the inability to breathe from your nose. Other signs include a crooked nose, where you feel like your nose is crooked to one side or the other, frequent nosebleeds, crusting, whistling noise, or frequent sinus infections.
Now, the most telling sign of a deviated septum is that the obstruction is fixed. It means it doesn't get better. It doesn't get better whether your allergies are getting better. It doesn't get better in the morning. It doesn't get better with head position. Really, if you can't breathe through one side or both sides of your nose always, then there's a good chance you have a deviated nasal septum.
A lot of times people wonder why do I have a septal deviation. A lot of times people are just born with it. About 70% of the population might be born with a deviation in their septum, but it's not causing them any problems. However, they might have a subsequent trauma or they might hit their nose or that deviation can continue to worsen as they get older. So a lot of times people develop symptomatic septal deviations later on in life.
Nasal septal deviation can be fix through surgery. Whether it's in conjunction with a rhinoplasty or simply a septoplasty, really the surgical goal is to get a crooked septum back to the middle.
You might be wondering if you are a candidate for a septoplasty. Really the best way to know is a diagnosis. If your septum is deviated and it's symptomatic and causing you problems in your life, can't exercise, can't sleep at night, you're snoring, these are all reasons to have a septoplasty.
It's a same-day surgery. You come in, you go home the same day. You typically have nasal splints that are left in the nose for about seven to 10 days. Sometimes we might leave that in a little bit longer if the septal deviation was very severe.
In the postoperative period, the first week is usually the hardest because you have those septal splints in place. So patients kind of say it feels like the worst cold they've had. They feel congested. They feel like they have a lot of mucus. But at that first postoperative visit when those splints come out, patients feel a lot better and they can usually breathe better instantaneously. But it may take up to six months for you to feel the full effects of the surgery.

I need this surgery after 20 years of suffering 😢 my nose is always dry and stuffy. I have headaches everyday and I have sleep apnea . Does this cause headaches? I am so scared of the surgery 😢

hi dr, i have a question for you if you don't mind. in your opinion, in regards to nasal congestion, what do you think is a bigger contributor, the nasal septum deviation or the turbinates? thank you

This will certainly depend. If your congestion is related to medical issues such as nasal allergies, the turbinates are likely the bigger contributor. If you have associated sneezing, watery eyes, itchiness, and congestion, the turbinates likely play a big role in your inability to breathe. Also, if your congestion alternates from side to side, then it's also likely related to turbinate hypertrophy. If you have an absence of other symptoms, in addition to a fixed nature of the obstruction, you may have a deviated septum. In reality, septal deviations and turbinate hypertrophy often coexist. Sometimes im able to breath from from one side of my nose and sometimes im able to breathe from the other side so im not sure if i have a deviated septum

The chances are you might have a deviated septum. One side will be almost blocked by septum and the other side will have excess muscle growth due to more space that will eventually block the air flow.

l want to ask question dr l have chronic heyperthrophic rhinitis and ellergy l went many ent specialist and the doctors can tell me same word you didn't want any surgery you didnt have turbinate heyperthrophy and me l feel alot of pressure my nose congestion ,bad breath 🥺

Doctor a question. An otorronolaryngologist offered to give me a direct injection into the nasal turbinates to reduce inflammation, I would like to know how safe this dose is, if it has promising results or if it has adverse effects, thank you

I have a deviated septum. Instead of traditional surgery, I had non surgical rhinoplasty. They injected dermal filler and could breathe perfectly now

Return to exercise and weight lifting will vary depending on the extent of surgery performed. For complex caudal septal deviations, requiring rib grafting or extracorporeal/anterior septal reconstruction, the minimum recommended time off is 3 weeks from exercise. At 3-6 weeks, I advocate for low to no impact activity, such as an elliptical, stationary bike, or rowing machine. Light weights can be considered during this time. At 6 weeks, normal activity can resume.

Hello Doctor, it's been a year today since I received several punches on the nose which caused me a broken nose. Today my nasal obstruction changes from one nostril to another. According to you doctor, should I have a septoplasty, while knowing that the obstruction is not fixed to a single nostril but that it changes sides Thank you for your reply

Hi doctor have you done a septal perforation repair and lysis of synechia? Last year, I had a septoplasty surgery. Due to that now I have 5mm hole. I feel at times I cannot breathe.

Yes. Synechia refers to a band of tissue/scar that forms in the nose after a procedure or trauma. This is frequently removed (or "lysed"), during procedures of the nose and sinuses (as in a septal perforation repair). You can book a consultation going to my website here: www.nycfacedoc.com/

Yes, revision septoplasty is one of the most common procedures I perform. It is first important to understand the nature of why the first operation failed. Many times it can be due to re-deviation of the nasal septum. Other times it can be because of mismanagement/misdiagnosis of the nasal septal deviation. When undergoing a revision septoplasty, you should fully understand why the initial operation failed, and how to ensure it does not happen again.

Persistent breathing problems after a septoplasty may be related to a number of issues. Common causes include re-deviation of the septum, medical related causes to nasal airway obstruction (e.g. allergies), scar tissue, sinus related problems, or other causes to nasal airway obstruction. It is important to speak with your surgeon and be thoroughly re-evaluated.

Hi! Septoplasty refers to reshaping or modifying the structure of the septum. The septum is a cartilage that forms a partition between the left and right nostrils. The vast majority of septoplasties performed should not and will not alter the appearance of the nose externally. However, if the deflection of the septum is severe, that it causes an external deformity, then corrective surgery likely will alter the appearance, usually for the better. For example, a "saddle nose" deformity refers to a collapsed septum. This usually gives the nose an "indented" or "pushed in" appearance, particularly in the nasal bridges. Correcting the septal collapse and deviation should make the nose look stronger, and less collapsed. Other times septal deflections can be so severe that it can push the tip to one side giving the nose a twisted appearance. Septoplasty should give the nose a more straight and midline appearance. In the most simplest of terms, septoplasty surgery should not alter the shape of the nose, unless there is severe structural compromise to the septum and external nose. In the latter cases, the nose appearance could change, and typically for the better.

Hi! I am also about to undergo a septoplasty, so my question is if the surgery will also correct the outside of the nose? Mine is tilted a lot to my left so I can not breathe through my right nostril. Will the septoplasty also straighten the nose? Not in the profile area, but like making it more in center of the face? Sincerely a 16 year old

A deviated septum can certainly make the nose deviated. However, a septoplasy alone, likely will not make the nose entirely straight on its own. Commonly, people require an "open septo-rhinoplasty" approach in order to fully correct a crooked nose. I advise you discuss your particular needs with your surgeon, and understand the likely goals and outcomes.

Post nasal drip can occur for many reasons. It can be related to medical issues such as nasal allergies (irritants from the environment) that lead to increase in mucous secretions. Deviated septums may exacerbate or make worse post nasal drip. Other causes of post nasal drip may be excess adenoid tissue, nasal polyps, medications (including nasal sprays and antihypertensive medications), infections, pregnancy, exercise, senior age, and neurological issues. It is important to have a thorough evaluation with your specialist.
