Can I suggest the Consecration mod? Lol. Making the undead highly resistant to damage unless you soften em up with fire or blessed items could help crank up the terror, methinks. I'd also suggest playing the game in Hardcore or with a mod that gives you limited lives like Unhealthy Dying.

billydabongo, i wanna just tell you, check out the mod "scape and run parasites", trust me, all of this list falls in comparason of terryfyingness of parasites, play with it with some shaders, and it's a pure nightmare fuel. Just trust me, you will NOT be disappointed what this mod has to offer. And this feeling at the end of your head, that world you are playing, is slowly decaying and doomed from the start, things get worse every day
