I have been an RN for 15 years and LPN for 4 years prior to that. I am a psychiatric nurse, background working in various areas of psych in the hospital but currently working in outpatient psych. Well I started a new position about a few months ago. The nurse I work with who trained me is a veteran nurse (and I mean veteran nurse). I was assigned to give a long acting psychotropic injection that is given every 3 months. The problem is there was no documentation in EMR or form we document injections on of date injection was last administered! I combed through the chart (EMR and paper) and NOTHING! When I asked her when she thinks she administered it she frantically stated she didn't know and she thinks she just gave it not that long ago, and had the audacity to tell me to just give it. I said no I'm not comfortable administering because I need proof of date injection was last given. She got snippy and said she was going to give it since I'm not comfortable giving it. Well anyway she ended up administering the injection, and supposedly found the date the previous injection was administered. (Yeah Right!). Patient was fine, but what if something went wrong. I am so glad I use common sense and stand up for myself.

Kendra, I'm Cuban. I lived in South Florida for most of my life, and I recently moved to South Carolina. It's so different down here. I have gained a better understanding of what black people face because I feel the racism of Caucasians here instinctively. Even though I didn't have that training to dot my Is and cross my Ts, I have felt in my spirit that I need to watch my back. It's sad we have to live like that as minorities, but it's true. I feel like God made me watch this video, and it shows new nurses not to get too comfortable and don't be a people-pleaser. Cover your butt.

When I did clinicals at Vanderbilt as an RN, NO ONE mentored me. I would try asking questions, looking things up, with absolutely NO SUPPORT in those activities. The managers reminded me of immature adolescents and also didn't have good managerial skills. The nurses didn't support each other in learning/sharing what they knew which would have helped the patients so much. I got out of that place and when I go for my own health care I avoid getting it there.

I have been a nurse 18 years and I have worked at university hospital, nursing homes, travel nursing private duty nurse. I learned before doing or administering anything you are unfamiliar with check the hospital or facility policy and procedure. Sometimes there is a policy and procedure book or it may be online. Topics such as chemotherapy, first dose is administer by chemo nurse. I called oncology floor and ask if chemo nurse could come and administer first dose. Nurse told me any one could administer. I told her hospital policy is chemo nurse administers first dose. I document her name, asked to speak to floor charge nurse and eventually chemo nurse came and administered chemo. The policies are created because someone died from a situation. Policy is created to prevent reoccurance. Chemo nurse brought special equipment and supplies needed to administer iv chemo. Patient also has to be closely monitored during first dose. Just in case patient has an adverse effect.

100% nurse defensively. I worked neuro ICU and we were taught that when a patient needs any form of sedation in the scan or the patient needed monitoring, the primary nurse goes and a resource covers the patients on the floor if one is available. Also, a nurse of 2 years should not be resource yet. They are too young in their craft. G status or mastery level should be mandatory. I work in ER now and we are finally enforcing the practice that nurses who have mastery are the only ones who work in triage. 5 years or more in ED. Too much can happen.

I recently found ur channel, I'm gonna check out more. I'm glad you brought this up. I'm late to the table. But I read some comments, 1 particular pointed out, she made several basic mistakes along the way, and they questioned why others were of the mind that the hospital was more at fault than her. My opinion is overall I agree as far as nurse is ultimately responsible no matter the crap set up by hospital. That being said, yes dealing with this grievous error with ur board, the hospital, maybe even a civil issue I agree. My issue is that sacred line has been crossed with her case where she was CRIMINALLY held liable and sent to freekin jail. When I came up we were taught over and over and over again by all sides never hide an error.we all have and will make them. It's a learning and teaching opportunity to be able to drill down to root causes and be able to amend practice to reduce risk. I felt it was sacred anyway. And to be thrown in jail for a mistake is like a break in trust. For reals. If I have to go to work and worry about being incarcerated for an error if I had the option I wouldn't. I am not alone. Cops get protection from such things with qualified immunity. Judges and juries send people to prison and later find out they were innocent. None of them are charged and go to jail. This is just wrong. Yes, go get the serial killer nurses sticking insulin in Iv bags. But not this. I'm done. Thank you for doing this

What's nursing insurance? I'm in my early 30s and I want to go to school for nursing... I feel like I messed up and let time pass me by but I've always wanted to be a nurse so now I'm preparing myself for this journey. However, I've heard and read that nursing can be hard especially when you're working in the hospital. I know myself and the type of attitude I have and if someone disrespects me, I'm liable to wild the f out. So now I'm wondering is it worth it? My husband tells me it's never too late go in handle my business and keep it moving... Thoughts on this matter?

I remember this case became a heated debate in one of my nursing classes. Some people felt like it was more the hospital's fault for not having the policies behind it, but I proudly stated to those who had a different opinion that at the end of the day, you have to protect yourself because your license is always on the line when you work your shift. Regardless of the policies, when you become a nurse, you must adhere to the nursing code of ethics and always cross your T's and dot your I's. RaDonda dropped the ball so many times and was negligent. I know mistakes happen, but I am appalled that she had so many chances to prevent this. Honestly, I feel like the amount of sympathy she gets is insane because had this been a black nurse, she would have been hanged at the cross. I feel this could've been avoided if she had gone with her gut instinct of it not being the proper medication. I come from a family of nurses, and I learned from my aunt and mom to constantly question your gut instinct as a nurse, speak up for yourself, and always ask questions. I understand things get tough, but her case is what every nurse should think about every time they clock into their shift. I am SUPER AWARE every time I am in the clinical setting because my job is to keep the patient alive and safe!

Exactly!! This video keeps going on and on about how to protect yourself (we know this generally speaking), but a person lost their life! And nursing teaches you to stop everything around you when you are administering meds. If it were my relative, esp my child? Oh heck no! NO sympathy. If the hospital labeled the drug wrong, etc. then yeah. Edit to add: You can look up the actual court records. Radanda said she "couldn't find a scanner" well, you ALWAYS stop and find one. Second, she stated that she didn't even look at the front of the vial to look at the name of the med (like WHAT?) but only the back, reconstituted it as described, and told the cops she was NOT rushed, and not understaffed. She just choose not to stay with the patient. This patient died a horrible and slow, painful death. There are two year nurses that get this right everyday, even in the same circumstance. It has also been noted that this nurse is somewhat frenetic and "air headed". The only thing she did right was to come clean as the hospital tried to cover it up when she told them. But now she is on some stupid crusade on her experience and making money from it while focusing on "administration errors" instead of how absent minded she is/was.

This video was so helpful, there are no classes on how to keep your nursing license and that is much needed content. Also I saw one of your videos where you said your license is hard to lose and the nursing board finds many things unfounded. Unfortunately that definitely depends on the state because in VA, the state board doesn’t believe in Just Cause and I have witnessed nurses being suspended or reprimanded over first time offenses that harmed no one. You hit the nail on the head when you said it was the nurses responsibility to administer the med and monitor the patients reaction. I believe it was not an error to delegate this task to another nurse. It is basic nursing, it is required, and it obviously was not done. If we overlook the error removing the med and everything in between if rhodanda had taken the time to assess the medication she gave she would have noted the mistake. Vecuronium is fast acting which is why patients need to have a secured airway before administration. Even if she thought it was versed and checked to make sure the medication achieved the desired effect the mistake would have been noted. Instead the medication was given and the patient was left alone and was found 30 minutes later. That is why I believe the charges were warranted and the punishment, no jail time, forfeited license was correct.

Shoot, all sedatives require a response assessment. And pushing vec to boot??? That's pretty instant. @@rupawateefalcon9545 She was expressing that an experienced nurse may not have agreed to do what she did especially with a nursing student. Another scenario she mentioned was that the floor nurse would not have asked a resource nurse to medicate her patient since that is not continuity of care. We all understand a fatal med error happened because of the nurse's actions however the sequence of events prior to it were also problematic. Facilities have their own agenda but at the end of the day we are the keepers & maintainers of our licensure not them.

When I first heard about this case, my first thought was she must have gotten away with previous drug errors for them to take her to court. When I read the charges I completely understood why she was being held accountable in a court of law. I disagree with the "she was a scapegoat" narrative. There were too many opportunities for her to STOP and check.

I need some serious directions. I had a clinical instructor tell me, you need to be in the ICU. I work at a LTAC, with 5 patients. Multiple come from ICU. I think many seem like they should be on step down or medsurg. Most of my patients have a trach and have medications due mostly all hours. I am ALWAYS late on administering medications. My location of my patient all over, I am overwhelmed. Now it’s 1 year , but there are short cuts made to finish. I hardly able to review notes unless I start 45mins before. Most times I get no lunch. Only the charge can sit at nursing station. We have wows and sit in hallway. I barely can sit because of all my patients needs. Also, I can barely talk to families . I am believe, but I’m not experienced enough for this to be true, BUT, I feel like the patients are just monitored by MD. I feel like our patients care is reactive, after they crash to get help. Most time they are sent to ED. Maybe this the norm? I don’t know. SO, I feel like I can’t do ICU and I should leave floor nursing. I feel like I love love to learn and and have in-depth knowledge to help patients . I feel like I have no time to do this. Others are able to maintain all the patients and give meds on time. I feel like I’m just able to do this in floor nursing

Is a resource nurse the same as a float nurse?! That’s what we call them in NYC. My understanding is that a float nurse floats to one particular unit and is not sent all over the hospital to care for random patients as needed. That sounds VERY unsafe and I would have never agreed to it either. No time for a proper report or anything. New nurses feel intimidated and afraid to speak up. I always work from a defense stand point as well. There’s no loyalty in nurse and they will throw you under the bus with no mercy if that means covering their own ass .

Whats G status?

The one thing I will never do as a RN is give my patient a sedative or anyone’s patient a sedative when I do not know the health history and I can’t confirm they’re condition. You never just give a drug. What if they have an allergy you don’t know of? What if they are taking a different drug that interacts with one you’ve just given? You have to truly know your patient to give a drug. We can all learn from this. But also in her defense, having a student you’re teaching at the same time can be distracting! She should have said no to the nurse who asked to give meds to her patient who she wasn’t familiar with, and she should have known when to tell the student she needs to focus. We have to slow down and think critically :(
