Upper lip and smoker’s wrinkles
Due to the high UV index in Brisbane, removing lip wrinkles is in the top 5 most frequently requested treatments that I get asked. Treatments will always depend on the severity of wrinkling- in severe cases, laser resurfacing is needed for the best results, in some cases fillers like Juvederm, Restylane and Radiesse can provide an excellent result with no downtime.
What treatment options do I have for upper lip lines and wrinkles?
There are several options that reliably erase upper lip lines and wrinkles.
My favoured treatment for moderate to severe lines and wrinkles is that of laser resurfacing. Laser works be ‘erasing’ the lines and stimulating new collagen. Results are instant, but recovery times vary.
Another method to treat mild to moderate lines and wrinkles is a combination of Botox and filler. This method is temporary, and needs to be repeated every 3-12 months. The upside is that downtime is zero. I prefer fillers such as Restylane, Juvederm or Radiaesse to treat lines in this area.
Why is laser treatment better than fillers for moderate to severe upper lip lines?
In my opinion laser resurfacing is better than fillers for moderate to severe wrinkles due to the following reasons-
Treatment is permanent and does not need to be repeated every 6-12 months-unlike filler. This does not mean you will never get wrinkles again, but with laser resurfacing I can ‘take your skin back 15-20 years’.
Laser resurfacing gives a much better ‘blend.’ By blending I mean it can treat other areas apart from the lip. Most patients with lip wrinkles and lines will also have wrinkles on the cheeks, lower lip and around the eyes- laser can address all these areas.
Fillers ‘fill and hydrate the skin’ with a natural substance called HA or Hyaluronic Acid. Fillers do not stimulate collagen. Lasers on the other hand can also address sun damage and pre cancer cells, as well as stimulating HA and collagen by your immune system.
What type of laser do I use for treating wrinkles?
My most used treatment is a combination of both CO2 laser and Erbium laser, in the one session. This gives excellent results, but the downside is that it takes up to 10 days before skin recovers. Fraxel laser, in my opinion is only modest in improvement of wrinkles and lines. Up to 3 treatments are needed. The upside is that this is a safe treatment with little downtime- ideal for anxious patients. RF or RadioFrequency treatment is marketed as fractional resurfacing by cosmetic clinics. It is not. RF can stimulate collagen, however it s role is early scar remodelling and compounded collagen stimulation. RF by itself, will need 4-5 treatments, and the outcome will be mild to modest at best. Not my ideal treatment. If I perform RF it is with either the Instensif Endymed system or the Infini with PRP.
Why do filler treatments suit some people better than laser resurfacing?
Filler are ideal for fine lines and moderate wrinkles but most importantly in patients who cannot have the downtime associated with laser resurfacing. Fillers are literally a walk in walk out procedure, whilst laser treatments are conducted in theatre and will be associated with a healing time of 4-10 days; depending on the severity of wrinkles and lines.

I haven’t smoked in many many years but then came the info to use a straw when drinking to keep your teeth white! At this point of my life I am not sure which is better white teeth or less wrinkles. I also think it will be hard to find someone in my area with your skill and truthfulness. Thanks for showing what works. Thanks

Thanks for covering this subject 😀 Upper Lip lines are a big issue for me. Not a smoker! Years of chronic pain, drinking out of water bottles & genetics are the source for me. If I was in Aussieland I'd be a patient. I live minutes from Dr Lee's office, I will make appt at her office to look into the procedures you have mentioned here. Thanks again 😀

All my lipstick used to "bleed" due to smoker's lip-wrinkles (in my early 20s 😮). I quit smoking when I turned 25 and the problem slowly decreased until it completely disappeared. I am now almost 60 years old, and the problem never returned (no lipstick "bleed"). I'm so glad I quit smoking for so many reasons, and this is one of them. (Blackheads were also terrible in the years I smoked.) I understand all the causes you mention in this video... I'm just stating my experience. Hopefully it will help others. 💗

Thank God you quit smoking pretty young! You dodge a bullet for sure...I'm 65 and never smoked but my sister continued up until she was diagnosed with emphysema! COPD is NOT curable, blackheads no worries, they won't kill you Sis is 68 and on oxygen 24-7 and she's miserable! Thanks very much for showing these methods of smoothing out these dastardly wrinkles. Since childhood I've always loved to whistle and now it looks like I've been a heavy smoker for years because I have deep wrinkles above my upper lip. If, and hopefully when, I have the surgery or other treatment, I'm guessing I'd have to cut down on the whistling! 😆

true but the amount of straw use versus a smoker, daily is worlds apart. average smoker purses their lips several hundred times a day, while a straw drinker averagers a few dozen. you can put the straw on the side of your teeth to avoid purses.

ABSOLUTELY AGREE re lipstick theory. I was a professional Make-Up Artist for about 15 years and I never stepped outside my front door without a perfectly applied matt red lipstick. Apart from lip balm, for about 20 years I never wore any other colour. The upkeep of this was ongoing through the day, which was fine because I always worked in front of a great make-up mirror and lighting. I am now 50 and despite having incredible skin, I do have "smokers" lines (never smoked) and they just look so out of place compared to the rest of my face. I've tried derma rolling, regular peels etc and spend quite a bit of time/money on my skincare these days - to no avail when it comes to these lines. If I was still in the industry and earning $1,000 a day I would DEFINITELY have C02 laser resurfacing done but I'm a starving artist/stay-home housewife and mum and it's a luxury I can't afford - booooo! That said, if I ever sell enough art, I'm getting on that plane and heading your way Dr Lim (I'm just down the coast in Newcastle). Thanks for another great vid x

Women also lose collagen a lot faster than men due to the menopause. This accelerates collagen loss and therefore wrinkling too.

There are so many out there who only recommend the lasers they own, even if it is inappropriate for the condition. My pores have opened in the last year and it has literally gone from barely there to horrific almost overnight. Upper lip lines as well have gone from barely there to very prominent. I'm fair skinned and caucasion. At the very least I would ask, and it would be so much appreciated, considering my issues and my complexion...if you don't know any great doctors in my area, what would be the best procedures to look for when I select a doctor?

OK, after seeing needle insertion in upper lip with the tip then coming out the other side? No. Your technique needs a little work. I'm surprised you loaded this video with that error in it as it looks so bad to do that. Also, one of the main reasons women get wrinkles in their upper lip if they're not smokers, is from stress, and holding tension around the mouth. Effectively having slightly pursed lips a lot of the time (not relaxed). PS: Yes, negative response "Women talk more than men". You can't be serious making a generalisation like that?

I had a fractionated co2 laser on my face with just a numbing ceam, let me tell you how horrible anrd painfull it was. I told the droctor that numbing cream does nothing for pain, but she went and used it anyway. Then she used fillers for my wrinkles under my nose, more painful then you could ever imgine. You should tell patients how very painfull it can be if numbing creams do nothing for your pain. Why can't they use the stuff they use at the dentists for tooth extractions.By the way lidocaine injections for stiches on my head, don't cancel the pain. I could still feel the stiches . Do I have a problem with numbing my face. or what.

What about this. I've a line on the lower lip which starts at the outer edge and extends down almost a half inch down the chin. Showed up about two years ago. I've noted it tends to wax and wane in appearance... I've been putting castor oil on it which helps only a little. There are times the right side of my lips, or corner of mouth, feels like it got a very minor dose of acid. This waxes and wanes to and I've not been able to determine if it's something I'm eating etc.

@davinlim I have deep wrinkles from working in the sun for years without sunscreen and from smoking. I have then above my lip and both sides of my chin. I have good skin overall as far as texture and elasticity but the skin around my eyes and under my brows is getting loose and crinkles horribly when I smile. In my 40's I was told I didn't look my age but now at 56 I look it. It affects me in a way I can't explain. I had fillers around my lip line, above my lip and in my chin. It looked much better but they were still visible and one deep line was still visible directly in the centre of my cupids bow. It's all starting to wear off now. So, I CAN get rid of them? I have both static and dynamic.

Paused at 1:18. Yes women do talk a lot. Thankfully. When we have worries we talk them through. When we're down we tell someone. We share our emotions a lot in general. In Ireland we have set up "Men's Sheds" in support of male mental health. I also set up a "Hen's Shed" (only second in Ireland) in our community to support women's mental health too. Most men don't talk about their feelings and unfortunately the suicide rates in men are on the rise here. So if the lines above my lip are there as a result of benefitting my mental health in some way, then I am suddenly looking at them in a new light. I'm glad some of them are there.

Dr Lim, I don't think it is a proven fact that women talk more than men - the only "research" I've heard on that is in John Gray's Men are from Mars, but that "research" has been shown to be non-existent. In fact Dale Spender's research showed that men talked significantly more than women. So... link to research for this statement please??

I was addicted to using chapstick. Not anymore. I was told that in some people microneedling can damage tissue. Are there people with whom it's contraindicated? A reason I won't do botox is that you are injecting bacteria into your body. That can't be good for people who have immune system challenges.

We talk more than men because our hypocampus is three times larger than men. Its bigger because we are Constantly thinking- much more than men. It’s in our DNA. And it’s believed to be the reason why we have higher rates of anxiety.

Please disregard his comments about the origin of above-lip wrinkles; they result from decreased hormones, i.e., aging, and to some degree genetics, not from talking more or using straws, which are are easily proved as untrue. Such a shame to have someone like this contribute to stereotypes. I'm surprised any self-respecting woman would give this person half a pence.
