The main reason behind it is of misusing or using those led flashlights badly so always use them in proper way and try to keep them properly. If you don't do so it can cause these kind of problems which results in wastage of your precious time.

NEVER touch one of these or connect something to the USB port when charging as it puts the USB port and probably parts of the case on full mains voltage (Depending on your location 100V to 240V AC).

Ren Castro But either the GND or VBUS pin is driectly connected to mains. And inside practically all USB devices the shield (aka body) is directly connected to GND. That means the Shield is either at Mains voltage or about 5V less than mains voltage, depending on wich way around you plug this death trap in.
