Great swimmers and surfers can be at risk for drowning. Surfer who loses board trying swim back through undertow can panic. No swimmer whether Michael Phelps or Navy SEAL can beat nature when the swimmer has underdog odds. Fatigue comes real fast, you're spot on.

Just remember MARCH Clear the airway of residual water/secretions by suction Assist ventilation with BVM attached to highflow oxygen w/ airway adjunct if pt can tolerate And keep the patient warm I would also give a couple ventilations before even doing the adjunct just to get some oxygen air into the lungs quickly

Would you not elevate the head of the backboard to get the water down? Intubate and deep suction? Or nasally intubate and deep suction if they have a gag reflex? Not at all challenging your wisdom, but curious for my own sake.
