I can appreciate trying to keep it simple, but there's a little more involved in the sloping example because the parallel distance along the grade is longer than the parallel distance along the level horizontal distance by the reciprocal of the cosine of the slope angle. I would suggest to only stake in the two parallel form boards without the two cross pieces first, then do your first two layout marks on the form boards to establish one side. Then just either use the Pythagorean theorem or trigonometry to establish the exact location of the third corner, then the fourth corner is established with the parallel distance. Then you can put in the two form cross pieces at each corner exactly where they supposed to be. This eliminates all the wasted time going back and forth with the cross measuring.

This also assumes the top 2 are perfectly level,and the bottom 2 are also perfectly level......rarely the case on a slope.
