Hi, I just purchased a Paramed Sphygmomanometer. I am confused as to how to use the loop that is supposed to steady the stethoscope. Could you elaborate about the loop’s purpose?

Do you mean the loop that is on the inside of the cuff? It is designed to attach a stethoscope so as not to hold it with your hand. It doesn't have to be used. To place the stethoscope there, unscrew the head of the stethoscope from the tube, thread it through the loop, and then put the tube back on. This way the stethoscope will always be attached to the cuff. Not everyone needs this feature. But some who measure their own blood pressure find it useful. You can try and decide for yourself. If you still have questions or I did not fully answer your question - write, I will correct the situation :)

it's so helpful!! is it better not to touch the bell of stetoscope? cz when I touch the bell in order to get the proper and clear sound of korotkoff, i get hand movement sound instead so i can't figure out systolic and diastolic sound clearly. Need ur help. thx

Yes, correct. Better not to touch the bell of stetoscope. Once you have the stethoscope in place, do not place your thumb on top to hold it - it can add artifact. Instead, secure it in place your fingers or cradle it between finger and thumb at the base with the cord draped over the back of your hand.

Measuring a child's blood pressure requires using an appropriately sized pediatric blood pressure cuff. The remaining requirements are basically the same as for adults. Here's a step-by-step guide: | Equipment Needed | - Pediatric Blood Pressure Cuff: Ensure the cuff is the correct size for the child's arm. It should cover 40% of the upper arm circumference and two-thirds of the upper arm length. - Sphygmomanometer: Manual (with a stethoscope) or automatic digital blood pressure monitor. Steps for Measuring Blood Pressure | Preparation | - Calm the Child: Make sure the child is calm and relaxed. Ideally, they should sit quietly for 5 minutes before the measurement. - Positioning: The child should sit with their back supported, feet flat on the floor, and the arm to be measured supported at heart level. If the child is too small to sit, they can lie down with their arm at heart level. - Sleeve: Ensure the child’s sleeve is rolled up comfortably or removed to avoid constriction. | Applying the Cuff | - Select the Right Cuff Size: The bladder inside the cuff should encircle at least 80% but no more than 100% of the arm circumference. - Place the Cuff: Wrap the cuff snugly around the upper arm, about 1 inch (2.5 cm) above the elbow crease. The cuff's lower edge should be just above the bend of the elbow. - Align the Cuff: Ensure the artery marker on the cuff is over the brachial artery, located on the inside of the arm.

| Manual Measurement | - Locate the Brachial Artery: Feel for the brachial artery pulse in the inside of the elbow. - Place the Stethoscope: Position the stethoscope's diaphragm over the brachial artery. - Inflate the Cuff: Inflate the cuff rapidly to 20-30 mm Hg above the expected systolic pressure (typically around 120-130 mm Hg for children). - Deflate the Cuff: Slowly deflate the cuff at a rate of 2-3 mm Hg per second. - Listen and Record: Note the first appearance of Korotkoff sounds (systolic pressure) and when they disappear (diastolic pressure). | Automatic Measurement | - Turn on the Device: Follow the manufacturer’s instructions to turn on the device. - Start the Measurement: Press the start button and let the device automatically inflate and deflate the cuff. - Read the Results: The device will display the systolic and diastolic pressures.

A stethoscope is used to listen to arterial blood flow sounds. Use the stethoscope to listen for a thumping or knocking sound. When you hear the first thump, make a note of the pressure on the gauge. This is your systolic blood pressure. Keep watching the gauge, while using the stethoscope to listen to the thumping noises. As soon as the whooshing noise subsides, and you hear only silence, make a note of the pressure on the gauge. This is your diastolic blood pressure. You can see how to do this in the video. I have a question about wrapping the cuff around irregular or large arms that narrow down near the elbow... should you make the cuff snug along the length of the arm or keep the cuff wrapped evenly so it's loose near the elbow and tighter toward the shoulder?

For a correct measurement result, we recommend to put the cuff tightly. But so that the middle finger fits between the edge of the cuff and the hand. But don't compress your hand too much. Reason why radial cuffs are made. You would have to let the cuff contour around your arm for an even pressure the width of the cuff.

You should not take the first (systolic) reading when the needle jumps but rather when you hear the first faint tapping sound, known as the Korotkoff sound Phase I. The second reading (diastolic) is recorded when you hear the last pulse beat. An often overlooked aspect of using a sphygmomanometer is that when redoing a measurement, you must fully deflate the cuff to 0 mmHg before starting again. There is no need to remove the cuff, simply squeeze it with both hands to release all the air. If you don’t do this and instead pump the cuff back up to 160-180 mmHg without deflating it completely, subsequent readings may be inaccurate. In noisy environments, it’s sometimes necessary to repeat the measurement to ensure accuracy, especially for patients with very low or high blood pressure or a weak pulse.

I have never heard sphygmomanometer pronounced the way you pronounce it. To make sure I wasn't hearing things I listened to how it is pronounced. I've always heard "sphyg" pronounce like i as in it.

The measurement process is the same as shown in the video. When fill the cuff with air, keep the arrow about 160 or slightly larger. Then start letting air out of the cuff so that the manometer's arrow descends 2-3 marks per second. At the same time, listen to your heartbeat using the stethoscope. When your heart beats, the arrow usually makes a short, abrupt jump. If you have any questions, feel free to ask :)
