There is nothing wrong. But, it is always better to try others ways of beginning the purpose and try writing incorporating the main reason for writing the letter. I am writing to refer, is a usage that most candidates adhere to. So, if we can have a fresh way to start, it is always encouraged.

Yes, it's necessary to include the address of the patient as the person (reader) is accepted to visit the home.
