In this video, I demonstrate how to x-ray a shoulder, a routine shoulder 2 view minimum exam (internal, external, and shoulder-Y). This is my protocol at my facility. Other views can be ordered by the physician. I shoot my shoulder-Y's PA, as this is the view I was taught in school. It limits the radiation to the thyroid. I recommend getting used to the AP position as well for trauma cases.
I also recommend not using my "method" in your 1st year LOL I don't remember exactly when I started it, but I do remember trying it out once I got tired of going back for a simple turn of the hand. Try it out when you get more experience, and lmk what you think.
As this is my first positioning video, please give me some feedback on the layout of the video. I thought it would be beneficial for you to "observe", then "assist", and finally you can go out there on the floor and "perform" just like you would in real life.

Love the energy bro!! I'm a final year radiography student from UK. Be cool if you could do x-ray positioning videos relating to adaptive technique e.g. when the patients is in a bed/trolley and is unable to move independently; resulting in us having to position the tube/detector around them.

Hey Rey, can you make a video with an easy way to get the y-view on an AP projection. Most hospitals perform the AP scapular y instead of the PA

I can go over that in a future video on trauma shoulders. In the meantime, just use this video and flip the patient. The positioning and radiographic criteria and how you fix your repeat will be identical, just the AOI and "pivot point" will be the shoulder further away from the bucky

Great video. I've been a tech for about 6 years now and wondered when someone would highlight us on KZbin. Our students struggle with C-ARM and surgery cases the most, I've been thinking about doing some videos in the OR but I'm camera shy....could you do a mock surgery case?

That's an interesting way of doing it, Rey. Here where I live, some locations would even include a little bit of spine for them shoulders! And the Y we usually do it AP...👌🏼

love your channel, thank you! I work in an Orthopedic office and the shoulder series they want here is the Grashey in neutral and external rotation, outlet y-view, and SI axial. If you did a video covering those that'd be amazing. i am always looking to improve my skills.

AP shoulder should include the sternoclavicilar joint medially. Your projection did not.

I haven't reached clinicals yet but does your tube move when you move the bucky? all on it's own? We have an old school type of tube and bucky in lab and we have to move each separately.

Communication is key. Moving an injured patient siring words and not hands is a challenge we’ve all faced. From the looks of the X-ray taken, the way you’re moving him is just hurting him more.

Hey Rey! I’m a 2nd year student and my clinical sites protocols are very different, which I know every hospital or outpatient center is different but it’s interesting seeing something new! We do a normal AP internal rotation, then we do an oblique view or grashey with a external rotation. Then we do our Y-view AP instead of PA but I honestly think the PA is easier.

had shoulder x ray today(both shoulders are frozen due to mva) technologist manner does seem to make a difference.The guy who attended me was bossy and unsympathetic.has some questions about shielding but responded in annoyed manner.Is there a chance that due to annoyance that he might exposed me to higher radiation dosage by increasing MaS and ms excessively?is this a possibility,not saying he did it but no idea how much radiation can one get from it when used carelessly.

unfortunately we are all human and this can happen. Not giving the tech an excuse, but you can say this for any other profession. To answer your question, there is a chance, but highly unlikely for a shoulder exam. I wouldn't worry about it at all. I recommend you calling and filing a complaint, however.

Amazing videos!! The most helpful videos I’ve seen. I just got a job as a CT student that will also be doing x-ray at a well ranked institution in my state. I haven’t done x-tables in over 10 years as I’ve only worked in a clinical setting. I’ve been feeling anxious to say the least. I have to look through your vids to see if you have a x-table hip. Can you go over well known portable trauma views like C-spine?

Just wondering why the IR orientation is done portrait. Should it be crosswise for internal, external rotation and grashey to catch the sc joints and soft tissues of the arm?

Great video!! I wanted to ask if you have done an axillary for the shoulder?? Or a velpeau? I get a lot of humeral head fx and the DR asks for a velpeau. If you have any tips? I’ve tried modified velpeau but my image is blurry.
