Absolutely beautiful job fixing the lower eyelids of that large-eyed lady. That was some serious retraction. I cannot understand why there is not a consistent standard of care and surgeons like you are then tasked with fixing a wholly or partially avoidable terrible outcome. Still, I am glad people like you have the skills and courage to do it.

@@AmiyaPrasadMD Thank you! This video is about blepharoplasty scars. My upper bleph scar is very visible, though it's improved somewhat with the use of silicone gel. For an "old" upper bleph scar that's obvious, is there anything that can be done to improve its appearance (short of cutting it out)? For instance, will laser help? Thanks.

Hi@@catherineo8689, Laser treatment is a possibility for improving the appearance of a scar, as well as treatments like platelet-rich plasma (PRP) to soften the scar, but an examination of the scar is needed to determine what can best help improve its appearance. Doctor I watched ur video of whether we should do hair transplant at a younger age or not and said it's not advisable to do it in your 20s. Since I'm 21 years old, I was planning to do it but after watching the video I have some doubts. 1. I know hair is important for looks lifelong but 20 to 30 is the age where we date and marry right. So isn't it better to get a hair transplant at that age. 2. So my doubt is hair transplanted doesn't get damaged. That's lifelong right?. Only other hairs which are not transplanted have the chance of getting damaged. So it can be prevented maximum by taking finasteride right? So if I did a transplant and took medicines like finasteride most of my hair will still remain right? Also what percentage of hair will I lose totally after I reach an age of 50 if I did transplant in my 20s 3. I have been using minoxidil 1ml daily prescribed by doctor but not taking any vitamins but there is no much of a difference and my scalp is visible too much when light falls. Is there any other way to solve this. Or should I do my transplant at this age

Hi Harry Maguire, A hair transplant is more appropriate after age 30 for several reasons: 1) only 20% of men in the 20s experience hair loss, so men in their 20s keeping up with their peers with a lot of hair will expect more density from a hair transplant to match their peers with lots of hair 2) if you experience hair loss in your 20s, you have a very rapid progression of hair loss, so you are losing hair faster than it can be transplanted, also known as the treadmill effect 3) if you continue to have transplants in your 20s, you won't have any donor hair left in your 30s and beyond 4) the amount of hair density to match your peers lessens as you get older, as more men your age experience hair thinning and hair loss 5) if you continue to have hair transplants without stabilizing rapid hair loss in your 20s, you would have spent tens of thousands of dollars on multiple hair transplants, but won't have much hair in your 30s as your native hair does continue thin after a transplant alone, leaving only sparse transplant coverage. Great video, Dr. Prasad. My mom is 1 month post upper blepharoplasty (full incision) and the line of scarring comes to a “head” at the inner eye corner. It feels sphere-like, similar to a small pimple. Should these scar tissues be a concern? She is currently only using a silicone gel.

Hi @viluong2564, I suggest you have your mom follow up with her surgeon to determine what it is, but one month after surgery is still quite early in the healing process. My patients don't use gels or ointments this long after surgery, but she should still follow what her surgeon recommends as I don't know details of her surgery, or examined her issue. Hi Anu m, Ptosis means drooping eyelids because the muscle that lifts the eyelids is affected. Hooded eyelids are due to stretched or redundant eyelid skin, which is called dermatochalasis. Chin filler longevity depends on the type of filler used. The most viscous hyaluronic acid filler I use is Juvederm Voluma, which can last up to two years. I hope this clarifies things for you.

My ophthalmologist just told me don't do this procedure because it causes all kinds of problems. He says it causes thinning of the lids and in some people prevents eyes from fully closing. He said stop considering it. can you address this please? I have severe hooding as I have some Asian bloodline. Also why do some Doctors take out the fat in the lower eyes and others don't?

Hi mike byrd, While upper blepharoplasty does not cause your eyelid skin to thin, poorly performed surgery can result in the eyelids not being able to fully close, but that's due to the surgeon, not blepharoplasty as a procedure. Removal of orbital fat is the primary reason for lower blepharoplasty, but in upper blepharoplasty, fat removal is often not done in younger patients. I hope this clarifies things for you.

Hello doctor, I had an upper Blepheroplasty 2 years ago which has left a very visible scar though out the suture line, the scar line is slightly above my crease and extends outward making it visible while eyes open too..I’ve tried fractional CO2 and the scar has not improved..what are my options to get rid of the scar?!

Hi Charan Preet, There is normally a difference of height between the two eyelids of 1-2mm, which is unnoticeable to others. I suggest you don't pursue exact symmetry because it's unrealistic, and will only cause more frustration. Hi Pranmaliyan Abdul Rahman, Male pattern hair loss and scalp DHT-sensitivity are caused by genetics, not by any behavior. Since pattern hair loss is a genetic condition hardwired into the DNA, it can't be completely cured, but it can be managed to maximize hair thickness, and scalp coverage. Sir I am 23 years old. I am suffering from hair loss from last 3 years. A year befor I visited dermatologist and he recommended 2% minoxidil for 4 month and 5% for next 5 months 0.5 to 1ml per day . I haven't seen as such changes in volume and It was making my scalp worst. So I quit the minoxidil and started to modify my lifestyle. Hi Saurav Patil, I don’t administer PRP alone for hair loss because it has a limited effect on hair, usually just short-term hair growth. Also, the number of sessions needed does vary, and the longevity of results is unpredictable. Sir my dermatologists prescribe minoxidil5%and finostride1mg I used this medication for 6months I got side-effect from finostride my doctor stopped finostride drug they not given any alternative dht blocker they said use only minoxidil. sir May I go bold without finostride im in early stage androgenic alopecia stage 2

Hi thevigneshhh, Finasteride is more beneficial than minoxidil as it can slow down and stabilize hair thinning, and it can be used long-term more predictably. There are more common causes of erectile dysfunction which also affect men who are not on finasteride, including stress, excessive alcohol intake, smoking, lack of sleep, lack of exercise, an unhealthy diet, and aging. Often, men on finasteride overlook their lifestyle choices if they encounter problems, but a healthier lifestyle needs to be considered as well. I hope this clarifies things for you.

Hi Dado De, Sorry to hear about your problem. 1 month after surgery is too soon to address this as the area is still swollen, so a revision surgery would be inaccurate. Hi StephOceane, There are other options, but I would have to examine your scar before making specific recommendations. It's important to remember that the results of laser treatment are determined by the experience of the practitioner using the device, not the device itself. Other lasers may help, PRP, etc., but I do need to examine the scar, and your skin type to determine the best strategy for you. I hope this helps you.

Hi Stars, As explained in your other comment, facial asymmetry is normal, natural, and usually unnoticeable to others, so chances are this not a problem at that needs to be fixed. Also, at age 13, you are too young for cosmetic procedures as your face is still growing and maturing. I hope this clarifies things for you.

Hi Dr., I have mild ptosis (1mm) of the left eye; no surgeon in my country will operate on it because of their fear of over-correction. I am now considering an upper blepharoplasty for loose skin. I am just wondering if that will make the ptosis more obvious as there will be more visible eyelid?

Hi Santana Caipirinha, Were you diagnosed with eyelid ptosis with a ptosis examination? There is normally a 1-2mm difference in height between the two eyelids, which is within a range not noticeable to others. People with ptosis that have a blepharoplasty done instead may end up with the same appearance, but less skin as the muscle is unaddressed. If you do have slight ptosis, it can possibly be corrected with a Muellerectomy that doesn't involve an incision in the upper eyelid. doctor do you think oral finasteride works better than fin, i have been to topical and not seeing much results should i shift to oral?

Hi ye, Oral finasteride is more effective since DHT is in the bloodstream, so it is better to block it systemically. However, finasteride is a prescription medication, so you need to see a doctor for a proper diagnosis of male pattern hair loss before taking it. I hope this clarifies things for you.

Hello dr, I'm 31 years old.. I had unilateral ptosis since I born (left eye) and my right eye is normal. 11 days ago I did the surgery: -ptosis + Blepharoplasty surgery for the left eye -Blepharoplasty surgery for the right eye ( successfull and immediate result shown on my left eye ) but I notice that my normal eye dropped just like my left eye before the surgery, my doctor told it takes up to 6 weeks till both eyes become even because my brain doesn't know yet that both eyes should be even. Is there a massage or exercise you recommend to strength my eye muscle or I should wait? And when I should be worry?

Hi Jumana AlTayeb, I don't advise anything like massage or exercises that could affect your surgeon's work. I suggest you follow your surgeon's advice as I don't know the details of your condition, or your surgery. Hello respected Dr Sir I was 80 KGS but due to Corona virus I suddenly lost 20 KGS now I am 60kg because of this my under eyes are very sunken and hollow I am 20 yrs old Sir if I again gain weight will my eyes recover or I will need eye filler/plastic surgery.please give your response in detail 🙂🙂

Dear Dr. Amiya! I had my upper and lower eyelids blepharoplasty almost 3 months ago and at 6 weeks PO one eye has a hypertrophic scar at the inner side near the nose and later has webbing on the side too, today I found that on the outer of the other eye also get webbing and I feel tight when I open my eye. My Dr. told me to massage it and I've done it since then but nothing improve. What else could I do? Will continuing massaging will one day change? Hope you can give me some hope as now I feel so stressed over this issue and worry it might get worsen over time. Thanks in advance!

Hi Chanda Seng, Sorry to hear that, but a proper second opinion involves a physical examination of your eyelids before I can make specific recommendations. Without an examination of your eyelids, I would have to defer to the recommendations of your surgeon who knows details of your anatomy that I have not seen. I hope you understand.

Now I worry about the scar after revision. Please, advise what I should do to minimise the scar formations. Thanks!

sir i m on finasteride for a month, and i m shedding thick and very tiny (1-3cm) hairs majorly and a very few of my normal sized hairs. What should i think about this, can this be a part of shedding and finasteride is just pushing weaker hairs, and so i should optimistic taking this as a sign of its efficacy? what is your opinion on this? thank you

Hi ye, Some finasteride users do experience temporary hair shedding, which can last several months, and it can be a sign of efficacy. You should be discussing this and other issues with your prescribing doctor as finasteride is a prescription medication. I hope this clarifies things for you.

Hi M G, Facial contouring can involve different procedures like bone reduction, liposuction, buccal fat removal, etc., but like I said in your other comment, I generally don't recommend reducing facial volume as it will make you look older than you are. I hope this clarifies things for you.

I wish you weren't on the other side of the continent. I'll be in the position soon here I can do a few procedures. There isn't anyone where I live that is good. I used to work at a high end country club and all those women went out of town for cosmetic surgery.

Since it is a non-surgical treatment with little to no recovery time, and in many cases no additional treatment, you don't need an extended stay in the US.
