I had my first pap smear and HPV test when I was 20 and my doc said I was HPV positive and noticed some discoloration on my cervix so they did a colposcopy and biopsy. Then they turned up as "pre-cancerous" cells. My doc then said I was still young so I might just fight it off naturally so to get another pap in 6 months. The next pap said that HPV was gone and my cells were back to normal, but those were the most stressful 6 months of my life. So I can relate to what you are saying.

@Jake Dean If you're not a doctor, you should NOT be answering medical questions. You might not "see a point" but there are reasons to test for it. There are many different types of HPV, some are high risk types that are highly linked to cancer and it's better for your doctor to know whether you have one (mostly if you're over a certain age). Also, if a female is negative for HPV and they're having protected sex (note: condoms don't fully protect you against HPV but they do significantly decrease the risk), they can likely extend the time between their next pap smear if they want as long as their others have been normal (but talk to your doctor first, it depends on your medical history). In addition, someone who has a high risk type should discuss that with a partner before having skin-to-skin sex, in case the other person hasn't received the vaccine or the type might not be covered by the vaccine (remember that you're not supposed to get the test until age 30 in most cases, so this isn't something that teens and 20-somethings need to worry about right now). Hopefully soon they'll develop a test for males.

I know you did say that there are people in this country who do not have access to gynecological care but I just want to emphasize that for people who may have missed it. I am a Women’s Health Nurse Practitioner student in Georgia and in Georgia there are 79 counties without an Obstetrician. We have the second highest maternal mortality rate in the country. I saw a patient this week who drove 4 hours to get to her prenatal visit. It’s a bad situation.

I remember being diagnosed with HPV when I was 19 years old and my doctor at the time shamed the hell out of me for having it. He made it seem like I would have an STD the rest of my life, and that I basically should never have sex again. He did a LEEP on me after one positive HVP test. After that I started seeing a new gyno who was horrified/infuriated that he had done that. When I eventually got pregnant at 23, I had to have a cerclage done, which my doctor said was a direct result of the unnecessary LEEP. I wish I had had a video like this when I was younger (I'm 31 now).

Thank you SO much for this video. As a 21 year old who had 2 irregular paps, had an HPV swab come back positive and has had 2 colposcopies now to monitor the pre cancerous cells this is so awesome. I was terrified when I first found out what was going on but realized there's a reason this testing is done and it's to catch things early so we can keep an eye on it to manage it! My ob/gyn was SO helpful in explaining everything from CIN grades to my body being able to actually clear it on it's on. She completely eased any worries I had and made me feel very comfortable and honestly made me trust my body. Everything looked really good in my most recent colposcopy and I go back in 6 months for another HPV swab and fingers crossed its alllll good. Definitely something that needs to be talked about more and needs to be a normal conversation!!! Thank you thank you thank you. Side note: I think a video explaining the process of a colposcopy would be so helpful. I couldn't find one that helped me or made me understand and think it could be very useful to some people :)

I'm so grateful for planned Parenthood being an option for me when I was poor with no health insurance and suddenly had a very concerning sign of HOV/cervical cancer. I ended up developing close enough to precancer after 3 years of tracking for OB to recommend LEEP. This was before I was 30. The LEEP for me made dilation an issue because of scar tissue. My friend who had one had the opposite where she was at risk for preterm labor. I think the at home tests would be a good option for people with little access or who can't afford a trip to the doctor (or several). I was lucky to have PP nearby and qualify for financial aid. Not everyone has that luxury. But I also agree with you, especially with who they are targeting and making it more of a convenience thing than actual health care.

Standard testing has changed recently in Australia. Where it used to be women over the age of have a Pap test every two years, now it is women over the age of 25 have a CST (cervical screening test) which tests for HPV only every 5 years. Cytology (a test to look for altered cells) is an optional add-on which is not covered by Medicare anymore and would cost the patient ~$60. If HPV is detected, they repeat the CST in 12 months instead of 5 years to check for clearance. If HPV is still detected, more invasive testing is then carried out. Home kits are available for women in special circumstances, but they are not direct-to-consumer, you still need to go through a doctor to have it done and get results.

In Czech Republic you should go to your ObGyn doctor at least once a year and you'll have that pap-smear test etc (every single year for every single non-virgin)... We almost don't see there cervical cancer, but there is often caught pre-cancerous changes on cervix in younger population. If you take the pils you must go there twice a year - once "small" examination and after that "big" with pap-smear.

Catching up on some of your old videos after coming across your channel. I'm 30, and have yet to have a pap smear (Never been sexually active, so when I asked my doctor when I was around 25-ish, (maybe earlier?) she said we could wait until I was 30, unless I became sexually active before then. This was after I had a panic attack in front of her at just the thought that I'd have to have one) so I'm probably due for my first this year if she remembers. I still start crying immediately whenever I think about it happening (like right now, as I type this! haha). Thankfully my doctor is amazing, so amazing that when she moved to another city, I decided to make the 1.5hr trip to continue to see her rather than finding someone local. (I'm autistic and have severe anxiety, so change is hard for me, getting comfortable with a new doctor takes me years) So because of her awesomeness, I'm sure the pap will be as 'painless' (mentally) as possible. But I am definitely going to have a support person in the room with me (my mom) regardless. All that said, your video made me decide to google questions I should ask her beforehand, and that brought me to this list of tips to make it easier on the day of, which I thought I would share for anyone else out there who is as anxious as I am! www.self.com/story/obgyn-pap-smear-tips

@aeolia80
@aeolia80 4 жыл бұрын
Not a doctor, but after getting diagnosed with HPV and learning more about the virus and the vaccine, I'd say yes, boys should be vaccinated alongside girls at an age before they become sexually active. Boys rarely if ever show any symptoms of HPV, but they can be a carrier. And since there is no reliable testing for men for HPV (I've heard the only testing available for men can be quite painful for them so they don't really do it unless they're showing symptoms) men (or more likely those born with penises, trying to make things more inclusive but I always fail miserably, lol) can easily pass the virus without their partner knowing with them thinking they themselves are "clean". HPV is a skin virus that affects certain types of skin, not just the cervix, and men can show symptoms in those areas. HPV is not just a "girl" thing. So yes, boys, along with girls, should be vaccinated. I used to feel weird about the vaccine when it first came out thinking that it would disrupt a woman's reproductive system, but that's a misunderstanding of what the virus is and moreover what the vaccine helps with
@baileyj8354

In the UK, you are invited to cervical screening every 3 years over the age of 25. In my region, as well as many others, they do primary HPV screening and then don't even do cytology if negative. I believe if positive, they bring you back annually. Sadly, uptake of screening is really low at the moment. I wonder if these tests would help improve it.

I think the numbers are so low b/c it’s uncomfortable and a lot of women are scared off by it. (I hate getting it done. I probably wouldn’t ever if I didn’t need it for health insurance at work and birth control renewal.😒)

I got my invitation almost a year ago, and here in the Netherlands, right now, we can choose, at age 30, if we want to do it at home, or at the doctors, with an assistant doing it. We get a flyer by post with information on both. And if we test at home, and the test is positive, you have to have a test done in the doctors office anyways.

Hearing that it's so common makes me feel a bit better about supposedly having it in the past. Also the plan of action I was given for it, as someone under 30, is pretty simple and I feel also might make others less freaked out about testing positive. Since there was some unidentifiable abnormality and the best guess was an already cured HPV I was told that we would monitor it annually just in case anything popped up and that was it, nothing scary but nothing ignored either. So even if you get tested and it comes back positive it might just be a case of supervising it so you know it doesn't become something scary, and that's not really so bad is it? Still something you 100% need to decide with your doctor but it's no death sentence!

This makes sense. I guess a lit of people would have to worry needlessly. However, I can’t help thinking that these tests could help prevent infection. When I was single and looking for a partner, I was afraid that the person I would eventually meet would have a lot of HPV strands and infect me, and that I would get cancer because of it. When I met my boyfriend I asked him to test himself for STIs, but I knew that HPV was not among the tests he did. That made me worry a lot in the beginning of our relationship. If he had taken a test and it had been negative, that would have saved me a lot of anxiety. If it had been positive, we could have just waited a bit for the infection to go away. It’s just terrifying that an infection that no one knows about is spread around and giving people cancer. Hmm... I’m not sure what I think - I see both viewpoints.

I’m 22 and in the beginning of the year I was told by my gyno that I had an abnormal pap during a routine pap after having Pap smears before that come out normal and was also told I was hpv positive as well. I didn’t even know that they tested me for hpv it was just done and later I was given results I was totally devastated me and my husband were newlyweds at the time so this caused me tons of stress and anxiety thankfully my husband has been amazing and caring throughout the whole thing. It was just hard receiving the diagnosis while my husband was deployed it made me go into a deep depression I felt disgusted with myself and I was alone while my husband was deployed so it really put a strain into my mental health. I had a colposcopy done and later a leep procedure which those were hard on me as well to go through on my own. Thinking I would never be able to carry children after this or maybe lead to infertility worst case scenario scares the crap out of me. I can definitely see why you talk about getting tested in your 30s for hpv and not now in your 20s because of the stress it can put on ones mental health mostly since most of the time it will clear itself up. But I’m also kind of happy to know what I have and that eventually it will go away hopefully on it’s own. It’s made me more aware of my health and my body and taking care of it. Really enjoyed your video! Made me want to share my own experience with this. Thank you! And sorry for the long message and if I’m rambling 😅

In Australia, with the change from Pap smear to a CST, they have been able to introduce a self-collection option. You still have to see the doctor but you can take the swabs home and collect it yourself. It’s not ideal and it’s only for certain circumstances (it’s better than no test at all). Is that an option where you work? This option doesn’t seem to be widely known - not sure if it’s due to the newness of it, or not widely advertised, or not discussed - which I think could then be preventing people from even making the drs appointment in the first place.

@mkuti-childress3625
@mkuti-childress3625 4 жыл бұрын
Sarpreet Kaur They have done plenty of cost-benefit analyses on the vaccine, and they have found that a girl is more likely to have negative physical effects overall from cancer than from the vaccine. That means that cancer happens more often and/or the side effects of cancer are way more likely or way worse than the side effects and severity of the vaccine. In other words, people die or have serious health problems significantly more often if they don’t have the vaccine. When vaccines were in their infancy, the original smallpox vaccine (which was the first) actually only made the severity of the disease 30% less severe if a patient contracted it, and people could definitely die from taking the vaccine. But they still clamored to get it, because people who had the vaccine still died significantly less often. I hope you make the best decision for you or your family!

ilona taylor-conway it is not standard in the US. It is optional. I had it done at 25 before I hit the cutoff age. My doctor said she was having all of her children vaccinated for it too, even her boys. But it’s not a requirement.

when I have my yearly exam with my gyno I have to have an HPV test done with my PAP smear every time because I was diagnosed with strains 11 and 16 with minor cervical dysplasia about 4 years ago. I showed symptoms with strain 11 four years ago, which caused my doctor here in Korea to have me get tested once a year to every 6 months depending on my results. At my doctors they snip an extremely small sample of my cervix (really small, but she said a swab isn't enough), so home testing always sounds weird to me, like it wouldn't be good enough. Luckily my last testing a couple months ago was negative for HPV and normal for PAP smear and I don't have to go in for another year. There's a youtuber named Hannah Witton in the UK talked about home testing for PAP smear, I don't remember if she mentioned home HPV tests but she did talk about home PAP smears, and I wondered if this is normal for the UK? I always felt that if you have insurance or live in a country that has single payer healthcare (like here in Korea) why not just go to the doctor for your yearly check-up, where the doctor can actually look at what's going on? I mean for rural or underpriviledged areas these home tests might be good, I don't know, the whole thing just sounds a little sketchy to me. I don't remember though if Hannah said for the in UK if their NHS pays for these home tests or not,

My doctor said she wasn't concerned because I had several years of normal ones and she would retest me in 2020 when I come in for my yearly. It's good to know this stuff because when you hear it's abnormal, you think you need to immediately get something done for it. I'm a little less nervous about it now

HI Mama Doctor Jones! Another great video, I really like your content and the way to explain everything. I'm curious if you have concerns about the efficacy of these home tests. I just wonder if the results would be 100% trusted if one of your patients did have it done and brought it in to you. I have a hard time trusting results if I don't know anything about the lab processing them. Would you then perform a Pap smear with another HPV test? Would it all depend on age? Also, do you have concerns about patients performing the self swab accurately since anatomy varies slightly from patient to patient. Obviously, just another reason to leave this kind of thing to your OB/GYN. Thanks again.

I think it’s wonderful to increase access but I know so many people who only attend a physical exam when they run out of their prescription for contraceptives, and I wonder how it will affect the frequency young people actually attend annual or semi-annual exams when they can obtain contraceptives without them.

I did this once because I was in between changing doctors and would have to wait for a month or so. I didn't want to risk it (I use it to manage my periods/anxiety), so the online consultation was a huge help for me. However, I wouldn't replace the actual GYN annual with the online thing because online, the consultation cost about 30 dollars. With my insurance, it's free and so are the pills. And while I hate getting pap smears, I know they're important. I can, however, see some people trying to replace their annual with the online app --- I used to work in a GYN's office and we would have girls calling all the time, complaining about having to come in just to get a refill. Those annuals are important, people. They only last for like 10 minutes and you're out.

I hate annual exams, but I have an extensive family history of reproductive cancers and issues. Every woman on my mom’s side of the family (except my mom, she died when she was 42. It’s possible she would have developed any one of a number of reproductive cancers, but we don’t know) has had at least one of the following: breast cancer, cervical cancer, uterine cancer, ovarian cancer, and any other kid of reproductive cancer as well as endometriosis. I pay very close attention to my cycles, to anything that may be abnormal, and I bring it up with my doctor or clinician. I’ve watched my aunt battle breast cancer (she’s cancer free now!) and I watched my step mother die of ovarian cancer. I don’t play around with my reproductive organs, as uncomfortable as a pelvic may be, it’s a hell of a lot easier to deal with for a couple minutes (or hours depending on my mental health state before and after) than to go through cancer treatments.

The vaccine only is for high risk (cancer causing) hpv. So wouldn't have prevented warts. Also Depending on your age, you might have gotten the vaccine that vaccinated against fewer strains. Ok so I have a question. My best friend was diagnosed with HPV while she was pregnant because she had an abnormal pap. She lost the baby (no relation to the HPV) and they called her in for a biopsy to look closer at the abnormality (or something like that). The doctor was shocked the abnormality was not there anymore she said it cleared up on its own. Several years later my friend goes to another gynecologist and she tells the doctor that she had an abnormal pap in the past but it cleared up. The new gynecologist automatically says she has HPV and sends her paperwork without telling my friend the results of the new pap. She has been to several different gynecologist since and they all automatically say she has HPV the moment they hear she had a abnormal pap once. So now she doesn't know if she still has it if she has abnormal cells still or what because they are automatically basing her health on what she had in the past. Is it common to go off a older pap results. I was told there are many strands of HPV that go away on their own but some dont and she has been scared to death that she might have the strand that doesn't go away, even though she was told by one doctor that it did, because the other doctors she has been to wont specify if the new pap results are HPV positive or if they are basing their decision on the one abnormal pap in her past.

I’d like to mention that dysplasia, which you mentioned could be caught via a Pap smear, is also caused by HPV. In that case a negative HPV test would rule out dysplasia.

I see you posted this back in February. I hope you see this comment. I had a friend from my church who I had to take the hour and half drive down to the teaching college in our state to treat her HPV before she was 30. She was divorced from her first husband at that point and said he gave it to her. She later married her second husband but they only could have one child. Is it likely she did not have to go through that back then? or that is why they they could only have one child?

Doctors already don’t explain stuff to you when you have results that require further testing. Just palm you off with referral letters and appnts weeks down the line, leaving you to worry and research diagnoses in more detail online. But at least on your second appnt you may get a nicer person who explains procedures better and lets you know there’s nothing to worry too much about. I can’t imagine dealing with home tests and the Internet alone. Then starting from scratch knowing something is wrong. Because you know the doc will start all the tests over for proper results

There seems to be an at home test for everything these days. I'm in Australia where we get a smear every 2 years. We also got the new test a few years ago, so because that test of mine was clear, I don't need to go again for another 5 years. I had the vaccines at age 23. I would suggest every woman just go to her doctor.

Dear doctor. How do you feel that in Sweden they look for HPV is you have an abnormal PAP? I was relieved when they had looked if I did have an abnormal PAP. What is your view on this? Why they did look if I did have it is because then I have to go often than every 3 year.

I'm surprised these haven't taken off in Australia, although I do not think they would be beneficial. In Australia, standard paps are not recommended or covered by public health care before 25. This is a relatively recent and concerning change. My mom had a history of severe dysplasia in her early 20s (she doesn't have a cervix anymore). I'm 23 and I have never had a pap because it's not indicated for under 25s, regardless of family history.

What is your opinion about HPV immunization? I have heard that the shot has its own complications and disadvantages. Since the virus doesn't negatively effect most people, are we chosing to take on more risk by having the HPV immunization when it is most likely one we won't need?

while i understand how debilitating anxiety can be (and deal with it myself), refusing pap smears can really affect your health going forward--and at-home hpv tests are no replacement for pap smears. and if you're under 30 and order these tests and come back with a positive, you could sign yourself up for lots of unnecessary procedures at the doctor's office. a good doctor will work with you to figure out how to protect your health and complete the necessary tests and procedures in a way that will make you most comfortable. but not going at all could seriously impact your health.

I was the same way. Didn't go for 10 years. So scared. But going to the doctor (to my surprise) relieved a lot of my anxiety. It's not as bad as you think it is, and knowing that for a fact is such a relief. Won't lie, the way there, back, and waiting for results is agony. Getting clear results, and or a plan is so anxiety reliving. Like a new lease on life. Totally worth the anxiety. Better to have a week or two of anxiety, than your whole life. Or decade! Also seeing the doctors 'non chalant' face helped me too. You're most likely not an 'emergency' or or out of the norm. To see them shrug off my 'terror' actually helped soooo much! 😂. It was like thank the Lord! I'm just nutty/anxious and not dying! 😂
