As a doctor who almost made the horrible mistake of marrying another doctor, i can tell the reason behind Henry's infidelity is purely "professional". Op is probably way ahead of him at work and he wanted to "put her back to her place" by breaking her heart using the closest person to her: her sister. It's clear in the way he announced the affair and the way he regretted not cheating sooner. My ex married my best friend few months after losing a job offer to me and said things similar to Henry. These types of people only realize the reality of their actions after facing an actual threat aka the dad kicking his a$$. My ex faced a defamation lawsuit and almost lost his license.
