One time I made a monster that was what happened when the illithids implanted encephalon gemmules from Phandelver the Shattered Obelisk into a host. It went, poorly for the colony to say the least.

The DM in the campaign I am playing in used Shadows... Any player who has ever faced a Shadow should be sufficiently terrified of them when they appear. The Strength Drain Action of horde of CR:1/2 Shadows could easily destroy a lightly armored 20th level party if a DM so chooses.

The worst monster I’ve ever unleashed is a beholder with the body of a troll. Roll a 1d4+1 to determine what eye stocks it has but give it the trolls reach, and regenerate, and claw attacks. There is another variant that is part demon because one of the demon lords encountered them and now used an army of them in the Blood Wars.

once i asked if a t-rex that was being used by a demon to fight as a normal one or not 5-10 minutes later we were facing a D-rex also i play pathfinder and these sound like something the followers of Kaikyton might come up with given they like making merging things together as well

As Invader Zimm said, "worse or... better". The answer is both and terrifyingly so. As you described this new horror, my brain started building a survive scenario. Take AVP (aliens versus predator movie), toss it into underdark or extraplanar underdark copy, players must survive between a nurlreth & alhoon mindflayer as the alhoon & nulreth are hunting each other. That'd be a fun ad&d 2e plainswalker challenge. Thanks for the new horror, and for continuing the lore.

I made a grey jester that had accumulated enough power to ascend to a khayal like being. The Ringmaster had the powers of both creatures plus additional reality warping powers. It also had grey jester minions in its Mirthless Carnival.

So, another interesting thing about D&D logic is that you can always pile some other layer on top of those you have. For instance, sooner or later one of those yts will acquire the status of deity. By personal achievements, leading to nations of worshipers. It is bond to happen. Even easier than that, some of them will find their way into vampirism, or graduate into liches. Nothing is too good to be improved by an upgrade to lichood, save for liches themselves.

That is something we can only find in D&D! Because they have such a deep and rich variety of creatures they can cook amazing combinations like that. And have a context where they integrate properly. However, the Human Genome project was finished successfully a long time ago, and now we got AIs able to deal with those inhuman volumes of information involved in dealing with DNA. Maybe we will see yt nurlreths walking around in our streets before XXI century ends. I am not thinking about the magical, and human-brain eating, faculties of those nice dear fellows. Only considering the "cosmetic" traits, such as number of limbs, facial tentacles, moisturized skin, that sort of thing. No spell absorption, unfortunately. Still, will lead to some interesting conversations. Some of you refusing to stay in the same room with one of your grandchildren because the scoundrel decided to have his body reshaped in that cute vintage shape, with the wrong number of limbs, and fingers in each hand.
