Tbf sane doesn't necessarily mean happy/healthy. Depressed, anxiety etc is very different delusions or psychopathy which this tierlist seemed more about. Dan Heng is the perfect example, he went through a lot of messed up things and that definitely impacted him but he is aware of it, knows how to deal with it and can mostly still live his life like he wants to. He is sane, tho not necessarily mentally well.
