Dr. Teal thank you, again, for this review using full procedure notes. Your technique in dissecting (is that a Root Operation? LOL) the notes is a great tool in analyzing the necessary information to build the PCS code. I'd like to see a couple of examples of procedural notes where the Official Guidelines come into play.
