This is our first video of The ICU room "uncut" academic discussions;
dog bite and rabies immunisation
These are uploaded "as it is" so that everyone can know that learning process remains the same everywhere; only crucial points are discussed here, rest you can refer and read from books;

The ICU Channel is a project by Educational Society of Bedside Intensive Care Medicine (ESBICM) for providing forever free bedside intensive care education to doctors and nurses working in ICU & ED. Before proceeding for immunization get history about When did dog bite occur. Whether it was provoked or unprovoked bite Whether dog is pet or stray, if pet whether dog was immunized or not. If dog is pet and immunized then no ARV If dog was stray whether dog is traceable or not. If traceable then observe for 10 days and if dog dies in 10 days then 5 doses and if survives ,3 doses of ARV. Then we categorize before deciding whether active or both active and passive immunization is required.

Hi. My puppy(unvaccinated) bite my hand while playing and the next morning my arm got Aching. I had tetanus shot. After i went to doctor he said it's no need for rabies vaccine. After three days I got high fever/headache and pain in my arm. So I went and got anti rabies vaccine (3 days after bite) . It happened in 2016. The puppy grew, gave birth to puppies and it was alive and healthy for like a year. But I still get pain in the arm (where I got Bitten) along with fever and headache every month for days. It's been 7 now years now

One doubt cat 1- licks on intact skin membrane - wash wound cat 2- nibbling of skin with no blood - idrv+ wash wound cat 3 - blood exposure or licks on mucous membrane - erig+ idrv+ wash wound.

Theoretically, it can survive in water in sometime and infect … but not get an authentic literature on this . Usually the virus doesn’t survive in water for long .

If wound is of extensive ...does antibiotics are required & what about dressing of wound sir???

Wound should be left open or with minimum closure . You can give simple antibiotics if would is large or dirty

sir ..inspite of active and passive immunisation ...we lost one patient wd rabid dog bite, bitten 1 month back. no history of missed doses ...what might be the reasons for that??

Sir, good discussion. One doubt sir rabies virus is neurotropic so it is more invasive in highly innervated areas like hand and face and not vascular areas, please correct me if wrong.

Sir what about wound management? Sometimes wounds are so large & deep like over on thighs, calf and face . sir do we have to suture them on that same day or have to wait for some days ?

That you have to take help of the surgeon. usually keep open or do the bare minimum which is required to achieve hemostasis. at times just dressing up . like the kid had fracture, we did the dressing, and after 24 hours, went for surgery. that's the best to my knowledge... still have to look into literature
