It seems we are using a multitude of studies, charts, graphs and the fallacy of developing the perfect evidence based system to keep inequity in place as long as we possibly can.

I think this is missing the very important measure of proactiveness instead of reactiveness. Resolving issues before they even happen is the best type of healthcare, should be part of the measurement system, as overall numbers will drop, and that's good for everyone. If that cleared 30% of cases before they even happen, wouldn't that be a good thing. Even if it was 1%. But if it's not measured, it won't improve.

The 6 dimensions do encompass proactiveness and preventative care. This would fall under the category of efficiency (it's also much cheaper to prevent diseases and illnesses than to only engage reactively to treat them once the patient gets them) and under patient-centered care, where a patient is able to receive care whenever they need it, continuously, and not just face to face visits. Read the Quality Chasm report and it will explain it all.

Safe, effectiveness of drugs don't over use drugs, patience,timely (avoid delay), effectively (stop wasting things we waste things alot in health care. accuracy

The USA does not have a health care system. It has a health insurance industry. To say that the health insurance industry is a health care system is akin to saying that your auto insurance is your car care system.

quality has not been defined here. quality is not the absense of harm but seeking the good which must be volitional ie free. dr.s must set aside their rational self interest,which is used to control them and replace it with virtue.
