Emphasis on systems is the antidote for guilt/devestation/shame. I love this. We should all use our mistakes as not only an unfortunate learning period, but as a way to be an active contributor building prevention frameworks that will save others from that mistake.

Totally agree.. and we need to set check points in the system to be able to detect those gaps in the system and fix them in a timely manner.

Errors happen due to complexity and chasm of Healthcare system..system re-engineering is needed when same mistake is repeated We use in Egypt kcl safely since decades..

listen again, the system is what to be discussed. Human Factors are important. To this day look-alike vials are placed side by side, despite the negative outcomes. Look for trouble and you are sure to find it.
