So the story of the game is: You play as a delivery man who accidentally discovers the truth about a certain apartment neighborhood. The one who wrote those letters on the doors in the red/alternate world is Kohei. Kohei has moved from his previous apartment to a new one because people were bullying him for his disfigured appearance and his new apartment isn't any better. We also see the backstories behind each letter corresponding to each neighbor and the ones from Kohei's mom. My theory is that Kohei finally snapped and did some horrible things out of anger.

THEORY: When you re-delivered to those apartments, each one had a note about them saying something mean to someone. In room 404 they died from a knife. The guy who came at you in the elevator was probably the guy who wrote the notes, which would most likely be Kohai because his mom wrote notes to him and one of them mentioned him getting made fun of I think.

I believe I have an explanation for those who might be confused. I haven't seen the other ending, but I did pick up on some stuff that Jay didn't in the moment. So, the character we're learning about is Kohei, a man who is alluded to have to disability or disfigurement of some sort. Judging by the last shot of ending 2, it might have been a problem with his spine (the fridge makes him hunch over, and his legs are bent in a way that shows he has a different center of gravity compared to someone standing upright.) This could be because of the weight of the fridge, but I think the fridge might be a metaphor for the weight his disability has over him and his life. So, Kohei is disabled and recieving welfare money from the government so he can live independently. At his previous apartment building the tenants bullied him into moving out, and this new group of neighbors isn't much different. They look down on him because of his disability, with the mother and other women seeing him as creepy and the "priest" seeing him as weak and without money of his own (showing that at least the "priest" doesn't consider the welfare money to be Kohei's at all). Kohei gets a dog, possibly either a service dog or just a companion, but that dog ends up a stray by the time our player gets to the building. Basically, the pictures of the fridge that line the walls of apt. 403 I consider to be the overbearing weight of this disability looming over Kohei. He cants make friends, he can't work, he can't even live peacefully because of it. Eventually, everything surrounding his situation finally gets Kohei to snap. Now, whether that "snapping" was truly murdering all the tenants that bullied him, I'm not sure. It would explain why the pets seen no longer have homes, and the fridge in the middle of the carnage being the catalyst of everything is telling. Or, because of the blooding dripping down into apt. 305's ceiling, Kohei could have committed [not alive]. It's hard to tell, as the murder scene could have been Kohei's fantasy and not the reality of the situation. Either way, I believe it could be up to interpretation. The other ending could hold something different for Kohei that I don't know. We'll have to see. TL;DR: Kohei is disabled and getting govt. Help, which the other tenants are bullying him about. The fridge on his back could be a metaphor for his disability. In the end, the murder scene could be either Kohei's fantasy as he commits [not alive], or he truly snapped and murdered everyone.
