I assume: "parent" means single parent (either mother or father who separated,), or unknown sex parent, or a parent who never gets married, but he/she adopted you. CIC updated this form in early July, but taken off after. But form IMM5707 there is still the column "parent".

Hello, This 5406, needs to be submitted in 2 languages (names) as you read. So, in my case, I first filled in English, got the print singed. Then I removed names only and printed again. I signed this as well and scanned . I combined both and uploaded a single copy. That was accepted

Hi Rahul, I think a few people asked a similar question and you asked for what application but then they never replied. For Permanent Residency Application of Home Child Care Provider or Home Support Worker where you can apply to bring your spouse and dependent children at the same time you are applying for PR the Document Checklist IMM 5981 states in section 7: Additional Family Information IMM 5406 that must be filled out by both the principal applicant and the spouse (whether they are accompanying or not) however unlike Schedule A: Background/Declaration IMM 5669 where there are boxes at the top to indicate if you are the Principal Applicant or the Spouse this form has no such area. In the fillable boxes it only has Applicant then Spouse in the next row. Because Principal Applicant isn't stated does this mean the Spouse just fills it in as the Applicant in this case? Or is there a different form that's not mentioned on the checklist that the spouse is supposed to fill out? Thank you in advance for your help.

My dad has more than the siblings that could fit on the first page. I know I have to add another page but what should I include on the additional page. In a similar case, in the additional sheet, the form name and number where written on top. Then the section was added where more space was required. The application was successful

Hi Rahul I really appreciate this tutorial. Thanks. But I have few questions, it will be great if you will be kind enough to answer those. I also received the request to provide this form. I am single, no siblings, my father is deceased and only have my mom with me. But no where in my initial application information I mention that I am bringing her with me. I am applying for PR only for myself. I don't understand why I am asked to provide it. Also do my mother needs to do this too ( I am not sponsoring her or so)? I am so confused. I asked my friends who got their PR and they were not asked for this form. Please help.

If they are not applying with you then no need . If you are just sponsoring your spouse then your spouse only needs to fill not his relatives

I don’t think so . If IRCC has asked you to fill then it seems you only need. I don’t have the full text of IRCC communication to you so can’t say for 100% sure but this is what makes sense to me

Hello. My wife is the principal applicant, i am her husband/sponsor. We all reside in Canada. I am a Canadian by birth and our daughter is also a Canadian. My wife filled out the form as the PA, and she entered all the info for her 5 siblings, her mother and father. Her 2 eldest siblings are Canadians by birth. The application was returned as IMM5406 is "INCOMPLETE". Why is this?

While going through checklist form 5533 under signature section i found out that this additional family information form needs to be signed by me (principal applicant) and other members of principal applicant including mother, father, sister, brother.
