Fun fact. When I was a teen back in the late 70s I lived only a couple miles from Ft. Lauderdale Executive Airport. A DC3 took off from there around 7 in the morning and crashed shortly after takeoff into a small field just past I-95 and narrowly missed a set of houses and a small strip store that had a 7-11 store, a bar and a laundromat. I jumped on my bike and beat it to the crash site to already find kids and others taking pieces of the wreckage away as souvenirs before the authorities got there. All that was left of the plane other than small bits was the tail and wing tips. The smoke rose up in a black column and insulation came down making a metallic tinging sound as it hit the street by my house. The crash was only about a quarter mile from my house. I’ll never forget the sound of the engines roaring and swoosh bang explosion it made when it impacted the ground. I didn’t even know what kind of plane it was until later on the news they said it was a DC3. Turned out it belonged to drug smugglers that were forced to land years ago coming back with drugs from South America. After they got out of prison they somehow located their plane stored at Executive AirPort. They were somehow able to remove the unairworthy signs from the plane, get it fully fueled up, start it and get takeoff clearance and witnesses said they had to bounce the plane on takeoff to get it airborn but the engines never have full power and they never got above 500 feet altitude. Crazy.

Wow! And did you know about another DC-3 crash from FXE? I think this was 2005, a Super DC-3 loaded with granite struggled to take off and crash-landed on the the middle of NE 56th street, sliding it between light posts and homes. Crew managed to get out before it went up in flames, no one injured. But what a mess to clean up. One of the engines had seized and they couldn't feather the prop.

At the corporate job I had once, the Hot Shot pilot that didn't want any help from mechanics, pre flighted and didn't even see the pin wasn't installed at all. I watched him load, close doors and fire engines. When he had no directional control, he finally got out and looked, and connected it himself. That was an awarding moment for me.

Not sure where “pin pin” comes from, it’s the steering pin. Without this pin in place, the nosewheel is not connected to the hydraulic steering. This way, the aircraft can be towed without potentially damaging the steering system.

What they're talking about is the upper torque link pin, which is removed for towing. Push button, and locking balls fall out of the way, allowing the pin to be removed. Shown in video.
I guess 32 years is a pretty good run for a private jet. Glad no one was hurt

So much for a proper ground pre fight check. I’ve got sixty years of watching pilots doing walk arounds from single engine to airliners and in many cases there checks seem just cursory, routine and not in focus. If it doesn’t jump out to them then just move on to the inside. Biased, expectations causes blindness.

In such a scenario, having the engines at the rear well up from the wing level is a definite advantage. If the engines had been slung underneath the wings, the outcome might have been very different.

Personally, as an MD-11 Capt for close to a quarter of a century, I did the walk arounds for the FO because I’d rather be outside than sitting in the cockpit. Now of course, rain, snow etc…..not so much 😂. Heat? No problem. Many of the mechanics are still friends. Nothing happens without their expertise! This is on the crew if it’s a required preflight item and they missed it.

One of my Gulfstream aircraft types on my EASA/CAA engineer liicence is GIV. The steering scissor has to locate flush before the pin can be inserted fully. When the pin is fully home and button released,the balls should fully extend, locking the pin into position. The addition of fhe safety pin provides extra security. After towing, it is the responsibility of the team to ensure this is done correctly. Final aircrew walkaround should include a visual inspection that the steering pin is correctly seated and secure. Maintenance pre-flight certification is normally carried out prior to towing, so the towing team have that responsibility.

@@ricbarker4829. With the steering scissor disconnected the aircraft nose gear can rotate freely 360 degrees without any damage. It can be a pain to recenter the two links of the scissors. I had to pull on the nose gear taxi lights and wiggle it often. Sometimes it helped having a second person to insert the pin.

With the upper link disconnected, the wheels can be rotated to any angle . When scissors are connected the full steering system is mechanically connected which limits the angle the wheels can be turned by the pilot during taxi. The scissors therefore normally are the driving link between the selected steering inputs and the nosewheel.

The ground crew should have been taken to court over this egregious mistake and the pilots licenses suspended for at least six months for failing to catch the ground crews obvious blunder. If checking the pin was on the pre-flight checklist it's on the pilot but in this case only partially. Fortunately, the airport had lots of open space. i don't think this would have been survivable in many airports.

Capt. Should have checked after any tow Have to do it yourself! Had refueled put avegas in my oil tanks..R 2000 7m2 realized it after they told me didn't take much fuel oly 5 gal.spent all night chasing down oil drain oil tank,replace plug plus safety wire. Carried tools on ac. Shit can happen.

This video leaves a LOT of unanswered questions. What is a pip pin? WHO exactly is responsible for such a mechanical item? I would expect that's why there are experienced ground crews? This should have been WRITTEN DOWN and documented and SIGNED OFF by the pilot in charge so no "he said, she said" issues. Frankly given the "layman here" importance of this, the plane should NOT have been allowed into service. IN THE END, what was the insurance company's findings and outcome? Was the pilot reprimanded? Like I said, lots of questions unanswered here. Fortunately everyone got out okay. Sadly the beautiful GV-4 was 'recycled' at least.

It appears the pip pin was not locked into place as well as had no safety pin installed. It worked itself out during taxi and so the nose gear was or became disconnected during takeoff. Seems to me when the ground crew was unable to fully insert the pip pin the proper thing to do would have been to remove it and inform the pilot. In that case it would have been obvious there was a problem when the pilot started to taxi the plane, if the pilot had not fixed the problem.

I hope the ground supervisor was fired. Since it's the pilots responsible to do a thorough walk around inspection, they at least need to be reprimanded. Good job stopping without hitting anything and thankfully no serious injuries.

This has nothing to do with the ground supervisor. So no, he doesn’t need to be fired or reprimanded. This was 100% on the pilots. The ground crew belongs to a separate company. They deal with hundreds of different companies and tons of different airplanes with different procedures.

I’m not sure I agree it was the PIC or SIC’s fault to notice a maintenance action not performed correctly. I’d love to poll ten pilots to see whether they believe it was their responsibility to check the nose gear pins when there was no report of NG issues.

Pilot here who has flown this type of aircraft: 100% the pilot’s fault. This is very important to check on the Gulfstream. Also, the ground crew is not part of the same company who operates the airplane and the ground crew deals with many different airplane types with different procedures.

I’m not commenting on this incident or this crew, but yes, I check it even when the other pilot is going to do the preflight. Typically. one of the plots removes the pin after shutdown and reinstalls it before flight. It’s certainly appreciated if line personnel do it, but I’m still going to check it. Line crew is not maintenance. Preflight installation/inspection of the pin is not a maintenance item and line crew is not maintenance.

In airplanes, just like trucks, an inspection before flight is basically show biz for the public, I doubt most jet pilot even know what is a pip pin , the elephant in the room is , they are pilots,/operators, not mechanics. The captain and co-pilot will take some of the rap, which really contributed nothing to safety, it was a maintenance error. This apply all the way down to ultralites, with the exception of homebuilts where the owner /builder is supposed to know enough to sign off the airplane because they build it.

@@oncamerawithroberthenry531 Line crews are not maintenance, so basically , after a plane leave maintenance with a missing scissor pin or installed wrong , or a wrench left in the intake, no one knows till the plane is at take off. I have seen 737 pilots making a show of checking for wrenches in the intake after maintenance, but I have never seen a pilot asking for a zoom boom to check the number two engine on L1011 or DC10s after maintenance. I am forced to come to the conclusion it is OK for the number two engine to swallow a wrench.

That pip pin must be pretty important. That size aircraft has to be towed and that pin must have to be removed in order to tow it. Then the pin must have to be reinstalled properly to have nosewheel steering. I don't fly jets so this is new to me. I fly small Piper aircraft. But those that fly these birds must know that ground personnel are as bad as female controllers. I suppose if I were a pilot on one of these I would really make it a point to check behind anything anyone did to or around an aircraft like this. Complacency. Please don't let me forget that "Complacency Kills" in whatever aircraft you are flying. I really have to remember to do all the detail work or wind up in some sort of calamity as this one.
