It looks like a Blackout, Loosing propulsion and steering. I saw the video, I would say best practice was not followed. See blackout can occur for many reasons. but the way that ship was trying turn under the bridge, Usually a good captain will align ship in straight line under the bridge in case of a blackout.... My captain use straight line practice. he planed it with crew. There are many best practice. We can only learn now. and this video is not required from India Today, this lady knows nothing about what happened.

It was an Indian Captain and all Indian crew and there was a local pilot as that is the norm when you're entering and exiting a port. But both the captain & the pilot were on the bridge and ultimately captain is the leader so u can't just blame the pilot simply because he was american and u wanna blame them, not the Indian crew. But that would be when it was the fault of the crew, right now leading theory is that the ship lost propulsion, they tried to stop the ship and even dropped the anchor but couldnt change course and did issue warnings before it hit the bridge. That doesn't sound like incompetence to me

Yes . Poor maintenance of the vessel could be a ground for this accident . Possible design defect of the bridge could be a factor for its massive failure with a single contact of a vessel when this risk should have been factored in the design and construction. Port approach could still be a problem along with underwater currents changing course . Poor stowage , ballast issues might be a cause . Without investigations apportioning blame on the Indian crew supports the thought of bad application of nicety . Accident can happen anywhere and by anyone’s negligence.

The captain is required to have a pilot aboard the ship, he is not required to allow the pilot to control the ship directly. Of course, if he ignores the pilot and something happens, he's responsible for that as well. There's no reason to blame the Indian crew or the American pilots at this point because there's no indication human error was involved. It was a mechanical failure of some sort.

The same thing the ship's crew was doing: not a whole hell of a lot, because without power they didn't have the means of controlling the ship. No engine, no rudder, no bow thruster. Dropping the anchor was all they could do, and if they'd had another mile or two it might have mattered. As for tugs, it's actually extremely dangerous for the tugs in that situation. Instead of one tragedy in 50 years, we'd incidents of tugs getting capsized and 4-6 crewmen dead at least once a year.

Many people are ignorant to the fact that, when a ship comes near it's destination port, a local pilot will board the ship and takes control of it for the safe embarkation at the port. Same way when the ship depart the port, it will be boarded and steered by the local pilot, until the ship reaches the open sea. So in the Baltimore incident no one can blame Indian Captain or crew. Also in this particular case the local pilot cannot be blamed either, because the ship lost power and lost control.

Pilot or not, the captain is still legally responsible for his ship. The pilot is an advisor and is often given effective command of the ship but is not the final authority aboard. But you are correct about the equipment failure being the issue and not human error.

It was the American pilot, not the captain, nor any crew members who alerted the authorities about the power failure. That was his responsibility. The ship should not have been cleared to leave port because there were power failures whilst it was being prepared to sail, according to the man who worked on the HVAC. The investigation will uncover what went wrong.
