Infections of the Finger
Infection of the finger is common, and it can vary in severity. Serious infection of the fingers will require urgent surgical care.
Felon is a deep infection of the soft pad, or pulp, of the fingertips. It is usually the result of a puncture wound. Swelling or pus is trapped in the small compartments of the pulp or the tip of the finger. Symptoms include unusual redness or swelling, firm swelling, throbbing pain at the tip of the finger, or visible yellowish area of puss. If the infection goes untreated, it may lead to severe symptoms such as skin necrosis, flexor tenosynovitis, osteomyelitis, and arthritis of the distal interphalangeal joint. Surgery is the usual treatment in the form of incision and drainage of the felon. If there is no foreign body in the finger, you will do the midaxial incision or the “J shaped” incision, and you will leave the wound open. If there is a foreign body present, such as a splinter or a thorn, you will do the volar longitudinal incision. Try to avoid doing the “fish mouth” incision; it will lead to unstable finger pulp.
Symptoms of paronychia include swelling, redness, puss formation, and pain in the soft tissue around the nail plate. Treatment is antibiotics if the infection is caught early. Surgery is the usual treatment. Incision and drainage with or without partial nail removal for subungual abscess.
Herpetic Whitlow is a painful infection caused by the herpes simplex virus that usually affects the fingers or the thumb. It is seen in dentists, respiratory therapists, anesthesiologists, and toddlers (children who suck their thumb). Symptoms include swelling, tenderness, redness, fever, swollen lymph nodes, burning pain, and vesicle formation on the finger. It can be grouped together with inflammation and redness at the base of the finger. The fluid in the vesicle is usually clear (not purulent). The infection is self-limiting. Conservative treatments include antiviral treatments applied to the skin (acyclovir). Antibiotics are not used unless secondary infection is present. Do not do surgery, surgery can make the situation worse.
Flexor tenosynovitis is a relatively common infection of the hand usually caused by Staphylococcus aureus. It usually occurs due to prior penetrating trauma and infection. The index, middle, and ring fingers are most commonly affected. Symptoms include painful swelling of the finger that hurts worse with motion. Flexor tenosynovitis has Kanavel’s four cardinal signs: uniform swelling of the entire finger (fusiform swelling, finger looks like a sausage), the finger is flexed, intense pain when attempting to straighten the finger (occurs early), tenderness along the course of the tendon sheath (most important sign). If the infection is caught early, treat with IV antibiotics. If the infection is severe, do early open drainage of the infection to avoid skin loss, tendon necrosis, and osteomyelitis. Surgical incisions used to drain the flexor sheath infection. Use a midaxial or Bruner incision. Use two small incisions, one proximally at A1 pulley and one distally at A5 pulley. Use an angiocath for irrigation. Give culture specific IV antibiotics. Infection may spread from the tendon into the deep palmar space or into the Parona’s space in the forearm. The little finger communicates with the ulnar bursa. The thumb communicates with the radial bursa. The radial and ulnar bursa communicate proximal to the carpal tunnel. Infection may travel from the little finger into the ulnar bursa to the Parona’s space. Infection can also travel from the thumb into the radial bursa to the Parona’s space. Infection may cause “horse shoe” tenosynovitis. Infection travels from the thumb through the radial bursa to the ulnar bursa infecting the little finger. May need combination of incisions for drainage.

I just had surgery on my hand for flexor S. I was bitten by my cat during an abcess drain and it got infected. I couldn't believe how fast the infection spread. Monday night = cat bite, Friday night = emergency surgery on my finger and palm. One comment I overheard was, "we might be able to save the finger", that was chilling! I got more information from this video then I got from 4 days in the hospital. The IV antibiotics knocked me for a loop, killed off ALL my gut bacteria and precipitated an entire 10 day period of diarrhea, yeast infection and cramps. Great visuals and knowledge. Deep understanding. What would you do if you are 1 -2 days from doctor. A Canadian logger told me when they got poked with a fray of the steel choker rope that goes right through leather...... Get boiling water in one pot(it cools down in the mountain air), another of cold water. Infected hand into hot, when cool enough, then into cold . You're pumping the blood in and out as well as giving bacteria the message to get the hell out. It's worked for me many times since 24. I'm now 70.

I had a felon and to anyone who would need some info here is my experience. I applied a lotion prescribed by my doctor and it became softer with time. Hence I thought that i would burst it. As i was studying on my table, i took my pen, and with the needle(as it was not that sharp and i was less scared), I started *scratching* it *little to little* and gradually with focus on that point(where it was yellow) all the green thing came out. Being grossed, i cleaned it with with the cotton. It started swelling. All phases came after that. Awkward feeling , swelling and the other day it started shrinking(I feeled very very irritated as it was very swelling)! As it was night already I decided to apply an antiseptic and sleep. It was the best decision. And the other 2 days it was same as that day (I survived that anyhow). After nearly about 2 weeks, It was all gone. Moral: Don't cut your nails with your teeths. Take a nursery manners book and read it:)

@@horrifier6608 as i have squished it, i would say the comfort you are getting now you would definetely not get after you squeeze that. But even if you leave it, it would take time. If you squish it, your hands would scream you to remove your dermis skin as it will feel itchy. Please don't remove that if you by any chance remove that. I would prescribe not to remove. As it will be getting older it would release those sebum and it wouldn't quite really swell. All my energies to you❤️❤️

Well. No diagnosis here... My thumb finger is swollen from the last joint to the end, I feel my heart beating in it but the pain is like a 1 even when I squeeze it and move it around. No visible bumps or discoloration. Ice water feels okay, but didn't reduce inflammation any .. slightly warm water feels like my finger is on fire. I'm trying a mild antibiotic cream but symptoms don't fit the bill for any of these... It's just swollen and heat sensitive. 🧐🤷‍♂️

I got a bone infection in my left thumb after breaking a small drill but and it going thru my nail into the bone, I put super glue on the nail to stop the bleeding, bad idea. A month later I couldn't take the pain anymore and was admitted to the hospital for 5 days and had surgery to remove the dead stuff and then Iv antibiotics for two months after. I'm a diabetic by the way and an auto tech, now on my right hand I have something in my ring finger that feels infected coz the pain radiating up my hand and arm feels a lot like the thumb I described before. I've been to two drs for workers comp so far and they haven't taken blood or anything. I go see their ortho Dr Tuesday to see what he says. Anybody with medical experience have any ideas on what to say to them to get treated correctly? They don't seem to be listening right now and now my whole body is hurting , I feel sick. They started me on keflex and gave a shot of rosephrin. I know what happened in the past could have killed me or at the very least lost my hand so I'm a lil concerned, that's what the drs said anyway

Thank you! I had a small incision in my right index finger the other day which was cleaned out and tested positive for MRSA. I was given strong antibiotic's and been feeling better.
