Its the fact Joker knows how to get under someone's skin, and pour salt into a wound that makes him truly insane and crazy. He's not inherently evil, but its also known that there is good in him just as much as how crazy he is. Like his saying goes: All it takes, is one bad day. And he proved that even the greatest super hero can turn villain, proved that it takes one bad thing to change everything. This is why the only superhero that can deal with mentally and physically with Jokers schemes is Batman. They are both crazy, yet understand each other so much that they will always be arch enemys.
