The other hole is missing some thread that broke clean off. Which would have been about the same shape as the metal shard from part 1.

Drill it out with oversized holes rethreaded drill out the key and you're done call it a day it'll probably work like that for its lifetime.

I bought one of these BCGs a few months ago and had the exact same problem. Didn’t cycle, no matter what rifle/ pistol I put it in. I didn’t do a reverse torque test (I wanted to send it back). FailZero sent me a brand new one a month later, didn’t have a problem getting in touch with them or with customer service. Haven’t had an issue with the new bolt yet (I have less than 500 rounds through it)

The holes are not chamfered as well. Any hole that will be tapped has to be chamfered first and I would say that is more of the cause of the top thread breaking than the coating.

Cutting a chamfer should go to all good metal. I would repair that.

Preface: hobbyist at best, not a gunsmith Late to the party on this one, but if one were to run a thread chaser or tap through those gas key holes, replace the gas key, secure with proper fasteners to torque spec, then stake the key as is warranted, would that not solve the issue? I wonder if the top of the thread trying to be unwound would be kept secure by the gas key holding it down.

Fail zero has a lifetime warranty! Why wouldn't you contact them for your client or have the client contact them for a warranty replacement before trying to replace the gas key and charging your customer?

Man $.25 worth of a chamfer would’ve gone a long way towards preventing that. I wonder if it was so unsupported because it was propped open with the shard that the screw elongated and just popped that piece out?

Well, I'm very impressed with your knowledge about the matters but, without trying to be a smarty, I would remove the tiny piece of loose metal and cut the correct thread size before getting rid of the part since I'm running on very tight budged and re-use whatever I can - take care!

Hmmm, no chamfer on the hole and the evidence that they attempted to torque it with contamination on the surface. I don’t think this was hydrogen embrittlement.

I've had 3 different failzero bcg,s and had absolutely no issues with them tested with 1000 rounds apiece in 5 different uppers during the tests although I would agree the quality control could be better but the whole reason I did the testing was I thought the coating was going to cause issues

Of the bolt carriers groups ive seen broken have both been nickle boron and were fail zero. One 556 bolt one 308 carrier cracked.

I have 2. One is ok, one will not cycle, fails to cycle when others do. Fail Zero refused to even respond. Both are nickle boron.

That looks to me to be the fault of the coating. The screw couldn't go in, but it was able to catch the top thread and shear it off. 58 in/lb might be enough to do that if there wasn't more than a partial engagement on the first thread. Without a full turn of engagement, you're torquing the weakest part of the substrate material on the entire bcg. Without 3 full turns, you don't have the full strength of the thread. It's a recipe for failure. I've done this in other circumstances,and yes, I've trashed the parts. I just ordered one of these and will be giving it a thorough once-over concerning the gas key when it arrives. If I find any similar issues, it will either go back or I'll also check out what's under that key.

I had what I'm assuming was the same issue. BCG wouldn't run in a blackout and after 2 mags in a 5.56 the key came loose. Fail Zero took it back and sent a new BCG, which I've had 0 issues with.

Curious did the owner send the unit back for warranty? If so did Fail Zero do right by him? It still does not negate the fact that the threading was undersized and the damaged metals. This would definitely fall under a quality control issue and if it happened to this bolt carrier wonder how many others from that batch were bad as well? Makes one wonder how this ever passed their inspections or if it was ever caught from batch inspection/testing. Any idea when this bolt was purchased? I know that's alot of questions you might not have answers for but would definitely give some closure. If Fail Zero swapped for another one if you can do a follow up video that would be great! love your videos. Very informative.

I believe that metal shaving could have put stress on the top of that rear bolt threads when it was assembled causing the crack.

NiB is a coating, where Nitriding is a surface hardening treatment. Think Chrome lining vs Case hardening.

The fancy coatings may be easier to clean but they can flake too.I use plain Colt bcg's and they have never let me down.

Wish i knew nickel boron was a gimmick when i first started building. I havent had issues yet with my toolcraft nib but i wouldve gone with phosphate if i knew half of what i know now

I have a FailZero with 1500 rounds on it and it's a much older carrier. I swapped it for a POF which I'm going to say is the only nickel plated carrier I trust so far.

@Concerned Citizen, cause you listen to this guy? I have 2 and they run perfectly. My competition buddy builds high, high dollar firearms from scratch (built a custom 1911 for Trump) and he recommends the NB Fail Zero BCGs. So listen to who you wish. I know from how mine perform that they’re good & then I’ll also take my gun builder buddy’s opinion over this Ya Ho any day. You have no idea about the background on this particular BCG that is being autopsied 🙄!
