IP security concepts-: what is IPsec peer?

Does an IPsec peer act as a single two-way connection or can it be in the middle of a connection between two devices?

For site to site vpn connection it is between two gateways i.e for two networks on both side. So you can say middle of connection between two networks and for remote access VPN it is between end user devices like pc and vpn server.
