My guess is that Sans purposely kept ketchup packets under his jacket and one in his mouth just in case he lost. Your first meeting with Sans he pulls out a physical gag with the whoopee cushion handshake prank, it would only make sense that your last meeting with him he pulls out another physical gag like faking an injury with fake blood prank (though it has another layer to it because he is actually injured but wouldn't normally bleed in such a way from it). In a way, the 'blood' ends up making the injury look less real since its inconsistent with the deaths of all the other enemies.

Medically transitioning is a separate topic from socially transitioning but sometimes medically transitioning can make socially transitioning easier or vice versa if OP meant let a transgirl medically transition, OP using "her" is correct if OP meant let a transgirl socially transition, then the context of knowing they will socially transition means "her" could still be correct, but "him" could also be correct as they aren't socially transitioned yet. I think more commonly "her" would still be better, but, I'd say just use singular "they" in this context instead. "Let them transition". Easier to use gender neutral pronouns than to get caught asking yourself "he or she?" Gender neutral language is always the best when unsure
