What's funny to me is I had gout and switched to Keto and I'm good now. In fact, if I eat processed flour on like a cheat day, it flares up. If I stay keto, my joints feel GREAT.

I used to get gout attacks up to 6 times a year; sometimes debilitating to the point of being mostly bedridden. That stopped when I cut refined carbs from my life. It's been almost a year without an attack.

Gout has different causes and cures for different people. For me the trigger was oatmeal. I used to eat oatmeal every day for decades, and had occaisional gout lare-ups. On my mother's advice I quit oatmeal 11 years ago. Zero gout flare-ups since then. I don't claim this will work for anyone else, but I'll take the results.

Also, hydrate really well. When I get dehydrated and combine it with too much HFCS or alcohol or meat, that is when I trigger it. But, over a gallon of water a day, losing weight, and lowering BP, I haven’t had a gout flare up in over a year. Was having them every few months before.

Yes you can do keto with gout, I had gout, took pills for it every day haven't had a gout attack but once in the last 3 years and a few months and the reason I did was because that day I kind of blew it at a family function and had a bunch of sugar, sugar gives you gout attacks, and haven't had one since again. This is a crazy acupressure tip for gout. I just post it in case someone is desperate enough to try. This is a method that comes from many generations in our culture. My roommate is Asian but he has been raised on antibiotics. He one day runs out of medication (that he ordered from overseas and couldn't get on time because of Covid but it's a blessing for him anyhow. The med will eventually ruin his kidneys). He was so much in pain one night to the point of crawling on the floor and finally listened to me. I told him to start from his groin, and go all along the inside of the leg all the way to the knee. You use your palm cupped up, the 4 fingers together except the thumb and grab the flesh as much as you can, close the palm and the thumb and pull the flesh out and then release. Go inch by inch up to the knees. Wherever hurts the most go back and do 7 times. At the start it hurts a lot and it's painful all along. Do for both legs. It is very painful, bruises but it does matter. He did that for an hour or more that night when normally you only need to do one pass and 3 times a day. In the morning he is back to normal, pain free and goes shopping. If you continue for a few months, your gout will be free. Sometimes he itches so much that he scratches until bleeding because of seafood especially shrimp or lobsters. You can check the meridian concept if you want to know why. For the moment do it for a couple days or a week and see how you feel. For those who have cramps on the fingers joints, you will realize also that the fingers will open and close more easily. You can also do the same on top of the legs from the groin to the knees, only this direction. Breathe normally. Don't tense up. You need the oxygenation to heal. This practice has never failed. I do it everyday for maintenance to strengthen the kidneys and liver. You won't feel pain once healthy. Pain means blockages. And we eat a lot of cruciferous veggies and avoid seafood. You also can use a hair blower and warm up the bottom of your feet before you go to bed and also in between your toes to calm down and rest well. And I forget..sit at the edge of a chair so you can grab the flesh deeply

I had a really bad gout flare up. I went to the podiatrist and she actually said to go on Keto and to look up Thomas online. The only time I get a tinge in the toe or ankle is when I’m forced to do dirty keto. Deli meat is insanely bad in my experience. Thanks for this video Thomas!

Couple of things; Meat is high in purines but so are a lot of leaf green veggies. You need to pick and choose the best and possibly limit your overall intake… Second, high protein foods are high in purines. This includes milk and milk products but it’s been pretty well accepted for the last decade that the purines that come from milk do not cause gout and in fact are somewhat protective. So load up on the cheese, preferably sharp and/or goat…

A strict Keto diet GAVE me Gout. I had never had it until a few months into Keto. I have since went back to a balanced diet with carbs, fat, and protein. I have had zero flare ups of Gout since. My weight is contained by using strict portion control.

But I'm talking about what happens when I'm using the same substantial amounts of real salt that I'm habituated to when I have days when I'm not in ketosis and I'm eating sweet potatoes, resistant starches and other healthy carbs. We need the sodium when we're in ketosis, heard that a thousand times. But what about when I'm not in ketosis. I've been on a ketogenic diet for years and I'm habituated to eating a lot of very high quality fats: organic, cold pressed avocado oil, pasture raised ghee and butter, macadamia nuts, olive oil, etc. That's fabulous when I'm in ketosis. But when I'm eating carbs in the same meal, what happens? Yes, I've heard him say you can gain weight but that is not an issue for me. What else happens? Actually, even the amount of coffee I became habituated to during fasting, when I was trying to lose weight, had to be drastically amended once I only needed to maintain.

Yep. First time I started keto, it caused me my very first gout attack. Stopped keto, it went away. Started Keto recently again, although somewhat reluctantly, but so far, so good. I have made some modifications - primarily a large reduction in red meat. Never want to go through gout again, it’s absolute hell.

Gout is one of the many downstream metabolic problems associated with insulin resistance caused by the chronic consumption of excess carbohydrates. A clean keto/carnivore lifestyle is the way to prevent gout as well as the entire panoply of downstream metabolic problems associated with insulin resistance. For many people gout is just the first arriving guest to be followed in time by additional metabolic problems associated with insulin resistance. I am speaking from a 30 year experience with gout that had transitioned into severe "refractory gout" that was longer responding to preventative treatment at the maximum dosage of allopurinol [800 mg/day]. The keto diet put my 30 history of chronic severe gout into complete remission within a matter of a few weeks. I have now been on the keto lifestyle for going on 3 years and have remained completely gout free as well as completely fixing my high blood pressure, pre-diabetes, obesity, and sleep apnea. My uric acid levels measured in blood and urine have returned to normal. All my blood work and vitals are normal. At 6'0", I also lost 75 lbs from 245 down to 170. Ironically, for complex metabolic reasons, the initial transition from a high carbohydrate standard american diet to a keto lifestyle can sometimes trigger a gout flare in some people. This can be prevented and managed with a daily low dose of colchicine during the first 3 or 4 months of the transition bridge to a keto lifestyle after which time the colchicine is no longer needed. I had suffered with gout getting progressively more painful two to four times a year for about three years. The last attack was triggered by too much spinach and was debilitating. I had zero attacks since switching to keto and sorting out my health about three and half years ago. In fact, the offending joins have nicely healed up and are smooth and allow full motion. I am very lean and eat more and more meat as I keep on working on my running and strength. I am over the moon and I am sure this will work for many!

When I 1st did keto and intermittent fasting, I had my 1st gout flare on the 3rd day. I watched a video and it said it could be due to a loss of potassium. 🎯 I have to take extra potassium citrate, so I won’t have a flare. Gout runs in my mother’s family, but it’s never an issue unless I add fasting to keto. I also found a couple of nutritive aides that get rid of it. The more gout flares, the more damage to your joints. Luckily, I’ve only had 3 minor ones in the last 1.5 years.

@Adam Dong I can’t really do keto anymore. I have specific arthritis symptoms that limit my animal protein consumption. I can only eat some fish. It’s an autoimmune issue. But, since I can’t tolerate gluten very well, I’m not gaining weight. I’ve actually lost a bit. I think I’m still staying within a good balance. But, I miss being able to up the weight loss with keto.

Before I started keto, I had 4 or 5 gout 'attacks' a year, which would practically cripple me for about a week and a half each time. During the 8 months that I was on keto, I only had a single attack, and during that attack I drank a lot of water, and had some concentrated lemon and lime juice for the citric acid, and killed it off in two days. After getting off the keto, another gout attack for 1 1/2-2 weeks, then they finally put me on the medication to reduce the uric acid, to hopefully avoid further flare-ups. Keto was certainly good for my gout.

I’ve had gout for years and always feel better when doing Keto. Sugar and high fructose corn syrup will cause a flair up for me. Apple cider vinegar and a lot of water was my go to for quick pain relief, so when I heard Thomas talking about the ACV health benefits I was already a believer. I will say there have been times I started to have a flair up because off the weight I lost. I’ve noticed flair ups might occur with changes to the body. I don’t think Keto causes gout. I believe sometimes people who are losing weight think Keto is the reason for the flair up, but it was actually the body adapting to the changes of the weight loss. That happens to me with any major change.

Over 10 years ago I was diagnosed with gout after 3 severe flares. I choose to change my lifestyle rather than take drugs againstmy Dr's recommendation. I increased my water and brewed iced tea (unsweetened) to 15 cups per day. Cut out all HFCS, took 2400 mg of omega 3 per day from fish oil. Reduced my body weight and gave up beer which was easy since I rarely drank beer and I don't drink liquor. Fast forward to January 2021 I switched to low carb diet in support of my wife which concerned me due to the possibility of causing a gout flare. I eat herring, sardines or mackerel every day, pastured eggs and some form of grass fed or pasture raised meat daily. I still take fish oil and eat low carb vegetables. Lost quite a bit more weight and fortunately no gout flares in over 10 years. My uric acid levels are still considered high however I attribute the elimination of HFCS, sugars and processed foods along with plenty of fluids and exercise to keeping my gout at bay. When I was eating a post gout high carb diet My blood lipid numbers were not great and now on a low carb diet my blood work numbers are the best they have ever been. For me I keep my carb intake to 10% or less of my calories on average, so I don't think I am considered keto. I truly believe inflammatory response has more to do with gout flares than consuming purines in the diet. Also balanced omega 3 to 6 IMO has a lot to do with inflammation. Per the "gout diet" sardines, herring are a huge no no yet I eat them everyday, they are loaded with omega 3 fatty acids.

I have been on pretty strict keto for % months and still getting flares. I am trying to figure my trigger for the inflammation. I am not sure if a weight loss attributes to that and how much.

Was 400 pds big, could eat anything, no gout. 6 months on keto, got gout. Ate alot of bruss sprouts, asparagus, red meat. Got on meds. Did omad/keto. Dropped 200pds. Uric acid levels are low now, but still have gout. Need to take meds. But can eat meat, seafood again. But not 2 days in a row or get a minor flair. With meds and strict keto you can dissolve your Uric acid spots. Also I read Uric acid is stored in fat. So when I lost fat, it triggered gout.

I did a bunch of digging into this subject 2-3 years ago. What I learned was (exact info fuzzy now) oxalates converted to purines to uric acid crystals. The kidneys can’t process the quantity of uric acid if eating high amounts of oxalates so it stores it in our joints. When a person goes keto with an emphasis on meats cutting out high oxalates your body will reverse the uric acid storage and will release those crystals which can cause a gout reaction. However, if one stays away from the high oxalate foods your body will eventually clear those crystals and you will probably not get a gout flare up after that.

However with that said there was a distinct relationship between Crystal formation, uric acid, and calcium oxalates. This would be a root cause of stored crystals in joints, as well as the two acting as substrate for stone creation. Perhaps you have a science background and can explain it better than me. @Adam Dong yes. Sounds like your body is dumping uric acid which can be dangerous however if you have been keto for 3 months you should have gone through the worst of it already. Check out Ken Berry’s videos some specifically on gout.

So this is a subject that I, unfortunately, know a lot about. I've had gout for the last five or six years now, as does my son who had his first flare at 24. My worst flare was multiple joints as well as the bursa on each side of my Achilles tendon, in both feet, at the same time. They were swollen to the size of footballs and so stiff that I could barely walk. I also had fever and chills with it too. I went to the doctor and and was told that "Keto is very acidic." so it made sense that this was the issue. I just smiled and didn't argue with her. What I have found to work for me is to limit myself to no more than 2 meals a day of chicken as my protein. When this all started I was only eating chicken and eggs as my protein, no beef. Also, I drink a lot of water. I do the old "divide your weight in pounds in half and that is the number of ounces of water you need each day" thing and that keeps me pain free most of the time. When I allow myself to get dehydrated, the gout always flares.

@Adam Dong I don't think that diet caused it, since my son and I both have high uric acid levels. I was keto at the time, and he most definitely was NOT. I do think, that diet and water intake influence it, though. I have noticed that if I eat too much meat or don't drink enough water, then I flare. So, my opinion is that my uric acid levels are high for whatever reason and I can control how that affects my body by what I eat and drink. That said, I recently found out that high uric acid levels can cause kidney damage, and since I do have diabetes, I have finally relented and am going to start taking a med specifically for high uric acid levels to protect my kidneys even as I stick with Keto to bring my blood glucose down and to loose weight.

I have hereditary gout, suffering from uric acid kidney stones starting in my teens. I'm in my late 60s now and I know what I can and cannot eat. 'I have reaction from beef, Italian mushrooms, (shitake ok), and uric acid forming foods. like high fructose corn syrup. I get good protein from fish, and add the veggies, eggs, etc. I do herbal and food medicines. Blood pressure drugs caused the worst gout attacks I've ever had. I have done fasting all my life, which helps.

I got gout over 20 years ago and take 300mg of allopurinal a day since then other than a 6 month period where I tried the tart cherry gout diet (it didn't work). Other than that one time I went off for the tart cherry diet, I've continued taking the allopurinal including for a 6 month and 3 month time period of being on keto. I had no problems with flareups while on keto and my bloodwork continued to show about the same level of uric acid as when I wasn't on keto. (All of my other bloodwork improved except cholesterol). I should also note that I am a heavy meat eater when on keto and when not and eat plenty of beef, chicken, bacon, etc. Also eat lots of eggs. The allopurinal was the answer for me and allows me to eat whatever with no restrictions whether on keto or not. Just another piece of data for the puzzle.

Interestingly, since starting Keto 22 months ago, I’ve had 1 gout flare up. Before, it was 2-3 a year for a week at a time. Now, I might feel a tweak every once in a while, but no flare ups. It’s been night and day compared to pre-keto.

I don't know much about gout but I was born with cystinuria. I had cystine kidney stones my whole life. The stones were so bad that I could've died from it a few times. I started passing stones daily, this went on for years until I started getting resistant kidney infections. That's when I decided to try keto. I started keto for MS, but it almost stopped my stones. I went from daily stones to monthly then to weekly. They're much smaller than they used to be. Still hurts but it's nowhere near as bad as it was.

Mice typically synthesize vitamin C so mice studies require taking this into account. Vitamin C is said to reduce Gout flare ups. So, perhaps eating foods like bell peppers that have higher content of natural vitamin C complex might help. This is not typically considered in keto?

I suffered with gout flare-ups for years when I was on the SAD. I started with keto first, and now do keto and ketovore interchangeably. No sign of gout or chronic inflammation since then, even though I am eating something with a "lot of purines". I know a good number of people who have ended their gout misery this way.

I used to have Gout flare-ups 2-3 times a year. I was put on Uloric years ago and it controlled the flare-ups. 5 months ago started Keto lost 40lb went off all meds including BP and Acid reflux meds and have not had any issues at all. I think the overall reduction of inflamation in the body with Keto is a big factor in preventing Gout attacks.

Since in keto I have done my blood work and me also have developed high uric acid. I limited meat consumption and it seems fine. Now I have too much bad cholesterol because I am addicted to cheese (even if I eat mostly goat/ sheep feta or aged cheeses, but I eat them daily) So now I am on carb cycling (complex carbs that is), let's see if my bad cholesterol goes away...

I suffered from gout in my feet before keto. I've only had one minor flare up since starting keto 23 months ago. I still keep my gout prescriptions just in case because gout really sucks. I'm definitely way better since going keto.

You hit the nail on the head, but it was noted by Dr Berg that the flair ups have to do with the glycogen that is being released into the body causing inflammation before you start depleting all the available amounts in your blood also before your ketones can reverse the root cause as you just explained at the same time as these uric acid triggers. Our family went on keto and 16/8 intermittent fasting at the same time two of us both developed gout, then after we got to our ideal weight it went away and has not come back for over two months. We did not change what we ate, and we have a higher beef seafood including salmon and other fish carsiferous vegetables in our diet.

in times i had low stomach acid, i had gout flareups if i failed to have ACV with my protein, ACV being the #1 clue. My hair was also falling out to the point of bald patches, indicating that protein wasnt being utilized. I concluded that my keto was too poor in nutrition to make stomach acid. I also had gout flareups when i fasted during this time (NOT good). I was having around 120-150g protein. Nowadays my stomach acid problems have been corrected after stopping keto. Im well nourished now due to inclusion of vegetables and variety, and i dont get gout flareups. I dont rely on ACV anymore and im still having around 150g protein.

@Adam Dong since making my comment, i have only had 2 very minor flareups that lasted a couple minutes each. My flareups were only a problem on keto due to the reasons mentioned.

My dad and oldest brother get the gout way to much. I just tell them that they're liver is over working from all the constant drinking, and that they're body is just using a toe or finger joint as a extra liver. It makes me laugh anyway.

Hey everyone, I’ve been on Keto for over 5 years with much success. However, I’ve been suffering gout flare ups off and on for over 20 years. After running out of creatine recently, I received a new shipment and took a dose, followed by an acute gout attack. As a result, I realized that creatine could be a major root cause. Therefore, I stopped taking creatine and never suffered a gout attack since. Lesson learned: If you’re suffering gout attacks and are taking creatine, stop the creatine and see if that solves the problem. Let me know, if that works for you. 👍

I had gout flare-ups for the second half of 2011, until March 2012. I had to use a walker to move around my home. I started keto, and no flare-ups in 9.5 years. I have had pressure in my big toe, when I cheat, but if I do Keto 75% of the time, and avoid too much grains, or sugar, I am fine. I still take Allopurinol, even though I haven't had a flare-up in so long. Fear of flare-ups, is enough to keep me eating low carb most meals.

A friend I used to stay with got gout in his ankle. I got online to find natural treatments. He wasn't on keto. In fact, he is extremely emaciated, but eats lots of sugar and carbs. I'm surprised he didn't get diabetes. He had to change his diet somewhat, but the main thing that helped the most was tart cherry juice concentrate about an ounce in a bottle of water every day, or maybe twice a day.

2018 went full keto. Gout attack once in May 2019 because we were on vacation for two weeks trying my best but could not control what restaurants used. Came back home and stick to Keto. Gout attack passed. 2019 till now, no gout attack. Eating more ketovore. A whole lot more protein. Don't fear the protein. Stop the cheat days.

@@lawbee2448 yeah, I tried but gave in to taco bell chalupa. Totally screwed myself over. I was also trying to keep it simple with a lot of bacon and eggs but I'm pretty sure they was using canola oil or some hydrogenated oil. Since then 3 going onto 4 years no gout. Just doing a lot of fatty protein beef and once in a while a cobb salad here and there. Probably the most sugar I'll have is from strawberries or blueberries.

It has a similar effect like allopurinol and also helps with blood pressure, I also take some beef organ supplements, and some keto electrolytes when needed. I usually do a 16:8 fast and once in a while 20:4 fasting. But my meals consist of some fatty meat of either beef, or pork. Once in a while some chicken but once in a great while. I will some times do some veggies if I feel like it. But all in all it's just beef, beef, beef.

According to other famous keto doctors on youtube insulin resistancy causes your kidneys to degrade and therefore unable to filter out the uric acid from the blood as well. When you eat a lot of high purine food you get a flair up. When you start keto also that can make it worse in the beginning. After your insuline resistance goes down and your kidneys get slowly better than you get better.

My gout activated when I was on Keto, but it runs in my family and it's not like it went away when I stopped keto for awhile. My Dr. said that my levels were so high that diet alone wouldn't bring my level within the normal range so I had to be put on medication.

Yet, there are people with high levels of uric acid with no gout and don’t we continuously generate countless quantities of purines? I don’t see how going low protein is going to make much difference. Besides, there are people on carnivorous diets who no longer have gout, so there must be another dimension to this. For me it was minimizing carbs and sugar. I did this first to lose weight then noticed my gout swelling going away.

The Australian beef industry is not as reliant upon grain as most others, also, our weather permits all year grazing. Snow country cattle just come down to the lowlands for Winter.

Gout, Uric acid build up is a direct effect of malfunctioning kidneys. Focus on kidney nourishing eating on top of the gout. Also I believe if I remember correctly calcium bicarbonate "tums" helps neutralize the acid a bit. If you have gout, chances are you have gut issues and wouldn't be surprised if you have GERD/ silent gerd, ibs or symptoms alike. Get a really good prebiotic, prebiotic, digestive enzyme, aloe juice, black walnut extract, wormwood, clove. Chances are, its starting from the gut with helibacter pylori messing with the acidity of the body. Be careful with wormwood as its hard on the kidneys" great for the liver and gallbladder though"

GOUT hit my toe a month after i started the Keto-IMF Diet. I was a very rare meat eater until i started Keto. Spinach was my mainstay before Keto. I've lost 10lb in the first month and lowered my blood pressure from 145/102 to 123/90.

I am doing a vegetarian keto diet with bonebroth plus salt and ghee, bulletproof coffee with mct oil and a pancake made of eggs and sunwarrior warrior blend protein powder plus water baked in rapeseed oil and veggies or fruit. No meat. This has decreased my psoriasis arthritis to almost a stop. The swelling in my finger joints is less, I can move better all over and no new spots have occurred so far. I am doing this now for 8 months. No carbs for me as I have intorlerance for all kinds of grain or pseudo grain and nightshades. Of course, I need to supplement many vitamins and minerals. But this works for me. Much fun is making my own chocolate with bio cocoa powder and cocoa butter and ghee plus various nuts and stevia.

I do keto and intermittent fasting. I have heard that fasting is more the gout culprit. Do you have information on tha? I have it in my family as well. I seem to have more flare ups when fasting. Also. I have to avoid vegetables with oxalates like spinach and asparagus. Those are the worst.

Take vitamin K2 with D3 to control the Uric acid level on keto. It happens normally in 1-3 weeks when you start keto but with Apple cider vinegar before meal with mother and vitamin k2 it backs to normal.

Last time I saw the inflammasome chain responses were highly correlated with high sugar consumption, because you're producing way higher amounts of ROS and then triggering the inflammatory response, so I'd guess that gout could have its origin on the combination between inflammation and urate crystals formation, for this reason a ketogenic with close to 0 sugars (mostly non-starchy vegeatables to get phytonutrients to combat ROS), moderate to low protein in the form of fatty meats and healthy fats like butter and olive oil would be ideal, in my opinion.

I switched to keto on June 24th after a gout attack. Since that I lost 26kg and my uric Acid dropped from 577 mcmol/L to 479mcmol/L. HOMA was 4.4 now 2.8.

From everything I've seen, protein as a percentage of diet is a wrong way to look at it. Protein as a base requirement (and adequate amounts, as opposed to excess or inadequate amounts) is a better way to look at that particular macronutrient. Of course that's similar to carbs. Percent doesn't make much sense. The grams you can tolerate and still be in the level of ketosis you desire does. This "percentage" thing can be very misleading. Back on track, I seem to have done better with MCT oil, more seafood and eggs, and less ruminant. I'm not sure if ACV at night and in the morning helps. I was dirty pescatarian keto for 4 years and had few flare ups. Then i switched to high carb vegan 3 months ago and have had 2 back to back flare-ups. So i guess my experience corroborates a low meat keto diet for reducing gout flareups. If i keep getting attacks, will likely go back to keto, and also looking for ways to minimize uric acid. Dehydration is a big one for me...

On Keto and IF 3 years successfully now. Down 40 kgs. All health markers excellent. My macros are at 30% daily calories from meats. Almost a cliche but it is consumption of fowl on Keto that triggers my rarish gout flareups (always in my feet). Stopping fowl consumption but maintaining caloric meat consumption (replacing fowl with grass fed beef) eliminates my gout flareups completely. My only downside to KETO ( but gout bouts were much worse pre-KETO): I love roast chicken and duck confit 😁😁😁

I appreciate the content. More than theory about this certain process in the body is triggered by this other thing but is balanced by yet another thing so the answer is maybe. The body and nutrition is so complicated. For me, I love to hear from people that lived it. If a bunch of people say they did keto and gout whatever vanished, that's says volumes more than studies or theory ever could in my book.

For a whole host of reasons, not least helping my digestive tract clear away micro plastics, I take a few activated charcoal capsules every morning. The more you can do to help your kidneys deal with urates, oxalates, as well as those pesky uraemic acid issues from purine rich foods the better. The more mackerel and sardines I can eat with this buffer, the better I believe. Though keeping things in balance the whole time.

Fat loss itself can trigger some people into getting gout flares. Supplementing with some amino acids might help reduce uric acid in the blood.

so i have gout, and i have non diabetic neuropathy, and? my gout is gone after going keto. most of my issues are gone after more then 25 years of suffering. all i know is i have not had a gout issue since keto and losing 80 lbs. off allopurinol, off thyroid meds, off htc, off everything except my bp meds. during childhood and most of my life i have had death threatening histamine responses. skinny or fat it is part of my life. i have to use as bi pap as i stop breathing 85 times per hour. keto has been a blessing. i just wish i understood why this change has made such a difference for my body. if i could can it i would be a billionaire.

oh and you should keep telling people. once your ketones become your source for energy? they are not high. my ketones test out low but i know i am in ketosis. how? when i am in ketosis my nose runs... constantly. too man carbs? my nose stops running. back into keto... well lets just say i gave up trying to keep tissues around. i carry a roll of toilet paper.

Most comments are saying meat isn’t the problem. Dr Ken Berry, a well known keto (and carnivore) KZbinr also says dietary purines aren’t the issue with gout. This is all very confusing.

it is... high uric acid not happens to everyone... genetic problem.. so meat intake still will add risk... just because someone people does not have goat attack does not mean their uric acid is normal... that issue still damage both heart and kidney.

I think it depends how" sensitive" your system is... Like if your uric acid level is already through the roof, I cant imagine dumping more purines can help the situation. However basic understanding is avoid all sugars and starches is best bet. And he's right inflammation is main issue so getting rid of all vegetable oils and grain fed meat is safest bet

I do daily 20:4 IF and when I made the switch from keto (20P/75F) to ketovore (35P/60F) in May 2019 it meant an 80% increase in protein intake, 90% meat around 2 pounds daily. I also reduced my Allopurinol by 33% from 300 to 200mg and over the next 4 months I saw my serum UA DROP from 5.3 to 4.9 so I dropped to 100mg and it rebounded to 5.3 over 5 months or so. I kept it there and it dropped to 4.9 again and it happily oscillated between 4.9 and 5.3 every few months. I have been completely unmedicated for well over a year and while it has gone back up to 5.9, I am not terribly concerned. My fasting insulin is extremely low at just over 2 and my HS-CRP is very low as well at 0.44. With the effect insulin has on impairing kidney function, I am of the belief that the increased purine amounts are easily cleared out when broken down into uric acid because my kidneys are working that much better. Having gone from a peak weight of 405 down to being weight stable for over 3 years at 180-185 it is pretty amazing to not also have gout attacks. It has been well over a decade since my last flare or attack and while allopurinol was surely keeping it in check when I was eating extreme SAD, the IF and keto/ketovore is surely the reason in my mind, especially factoring in the role of insulin and the studies showing insulin resistance being tied to purine metabolism.

I've successfully done a keto diet once, and for not just this diet, but any diet requires a lot of focus, time management, and patience. I went thru a 6-month strict keto diet with no exercise and lost 71lbs and 25% bodyfat. Yes, that's correct, no exercise. The disease that's making you fat is also the cure that can make you skinny, which is diet. Exercise is an ineffective health regulator against a poor diet. Keep in mind I've been a fattie for over 30 years, I was already 180lbs at 5'3" and 13 years of age. I started developing gout at the age of 23 and it has gotten worse every year. In my early 30s, I began getting flare-ups that kept me bedridden for up to 3 weeks. You don't want gout attacking your knees, both of them, at the same time. I'm currently in my early 40s, and gout flare-ups have subsided greatly because I've cut out refined carbohydrates from my life completely. When I attempted my first keto diet and successfully did it, my body did react violently to it because it was being pushed out of its usual comfort zone of carb-addiction. I went through 3-4 weeks of gout flare-ups on the keto initiation phase. I call it the "initiation" phase because it's exactly that, it's like boot camp but worse and for diet control. At first I consumed a bunch of protein with my high fats, but I didn't know what I was doing and was relatively new to the keto diet scene. I went through both gout flare-ups and the keto flu for about 5 weeks, once realized I was consuming too much protein which made sense because I was still getting gout flare-ups, I cut my protein in half and kept the fat intake high. After that, I entered the keto-adaptation phase where my body began absorbing the ketones it was generating. Remember, just because you're producing ketones doesn't mean your body is absorbing it. Keto-adaptation is a delicate phase where your body is absorbing all the ketones it will ever need, but you can be easily kicked out of it by eating too much carbs or protein. You'll know you're in a keto-adapted phase because you go from "hell hath no fury like a person on a keto diet" to "I think I'm high and I don't even smoke weed." A big difference between being in ketosis and being keto-adapted. I advise anyone who wants to go on a keto diet to exercise their due diligence and do their homework. So many scientific articles out there supporting the keto diet, and if you can include intermittent fasting in your daily eating regimen, even better. Also, expect to fail, that's the only way you will learn to do better. I work in IT, am a USAF veteran, and keto diet veteran and this was my $3.50 input.

Ironically the Ad for Matcha GT is counter to a gout friendly diet as Green Tea is umami and can increase uric acid. is that correct?

A big 'NO' to pea protein, for gout. I used to have bad gout, but not any more since going low carb. At that time, all the usual suspects were triggers... red meat, red wine, paté, prawns & lobster, legumes etc. When I went low carb/keto, I did so using mostly chicken, white fish, dairy, bone broth & eggs for protein. As a visitor one weekend, I got served beef three meals in a row.. I thought I'd be in trouble & braced myself for the pain.. which never came! I started cautiously experimenting, & now eat beef & paté & prawns with impunity as long as I keep carbs low, & frankly I feel a hell of a lot better for it. Pea protein is still a huge trigger. I made some protien balls to share on a walk with a couple if vegan mates... the pain was excruciating. If I'd been eating biltong or jerky I'd have been fine. I've since experimented, & pea protein is the very worst devil for gout, in my experience. I'm even very careful around pea & ham soup, or even just garden peas.. no more than a tablespoonful. Hommous is a no-no, too. I'm mystified when legumes are pushed for gout, they are just a rubbish food & trigger for pain, in my experience.

@Adam Dong, I can understand being scared. Gout is very painful. I find I don't get gout as long as I keep my carbs low.. I'm mostly not on a full ketogenic diet, just low carb. I centre my 2 or 3 daily meals around nutrient dense animal proteins & fats.. fish, eggs, raw Jersey dairy, as milk, cheese & yoghurt, red meat as lamb, goat & beef, poultry as chicken & duck, & pork, & some pheasant paté, sausages or pies for organ meats. I eat low oxalate veg & salad with those, & have some berries or other fruit sometimes with breakfast or lunch. between meals I drink water. I do eat stuff like rice or sourdough bread sometimes, or even cake, usually on weekends, or as a visitor. If I eat too many carbs, I get gout. On the rare occasions when this happens, I start drinking lots if water as soon as I notice the first signs, & also take sour cherry capsules every few hours, & until all signs go away. If it is very bad, I might also take scant prednisolone, the minimum possible, for a day or two, if needed. I hope this is helpful information for you. Kind regards.

I'm sure not knowing i had a Gluten intolerance had something to do with my gout. Since cutting out gluten all my ailments disappeared my gut improve sinus and allergies are gone not bloated anymore pimples are gone and so many others.

It’s funny you mention gout, I never had an issue with gout until I started keto. It made my one big toe and knee joint so painful I couldn’t move. It stems from your blood type and how you should eat. O blood thrives in a keto diet as A needs more carbs (not processed) from fruits, nuts, and say tofu. I switched to adding back more low GI fruits, nuts and small amounts of rice and less fatty meats and my gout went away and I sleep better. But with thyroid issues I seem to be having to worst time trying to loose weight no matter how I count calories, IF, keto, workout, I just struggle. Ever since I had my tubes tied and hit 40 I find it impossible.

I've been religiously doing keto and IF for a year and I'm NOT a big meat eater. I just this past week had a horrible Gout flare that incapacitated me for 8 days. I have decided to go back on Gout medication because I never want to experience another flare like that.

Never had gout...went KETO for a year strict...felt like a million bucks...went off the rails with diet, and got gout within a month...now on allopurinol. My guess is if you were at risk of gout (high uric acid) and went KETO, then went off...hello gout.

I had gout flare ups for 20 years, partially controlled by medication. I stopped eating anything with sugar 8 years ago and haven’t had gout since, despite stopping the medication.

Interesting, Thomas! We have exchanged posts on this before on Facebook threads. I agree low pork & red meat diet is probably good along with low shellfish. I am not saying none of these things but I think they are associated with high cholesterol if not purines. Good luck!

A couple other things would be to have plenty of fresh vegetables(celery, baby carrots, cucumber and bell pepper slices with humus around. And, when you eat meat, a ‘palmful’ diced with parsellaide (diced fresh garlic & parsley) is all you need with a little oil (butter optional).

I have been doing keto on and off for the last 6 months. I stopped taking my gout tablets at the start and haven't had a flair up yet. I get a shed load of fatty meat at the moment. Before I started keto I was starting to get gout in my hands (had it in my feet for over 15 years). Before if I wasn't doing keto and wasnt taking my tablets I would of had at least 3 or 4 attacks in a 6 month period.

As someone with gout (for over 10 years with about 2 flares per year) and someone who did Keto for 4 months I can tell you you definitely have gout issues despite feeling better than ever. My thumb felt like I would get a crisis any moment now but you never get the crisis. I stopped keto because I couldn’t get my Uric Acid under control, going to try it now with taking my meds daily. A little tip if you are going on Keto and rapidly loosing weight, know your Uric Acid will go through the roof as fat breaks down and extra purines are in the body. It was a surprising fact I learned from my doctor. So make sure you take your gout meds if you are rapidly loosing weight. For anyone that cares my uric acid levels normally are about 550-600, on keto about 800-900 that is without meds ofc. I don’t think the issue is the protein intake based on what I’ve seen I think it has more to do with the type of fats you take, for one I think you gotta scratch Keto Coffee of the list with MCT Oil. I know it probably doesn’t sounds scientific but I can definitely say getting the Keto Coffee of my morning routine did somewhat improve my tingly feeling in the thumb.The keto I’ve done is fairly limited in meats and my Uric Acid was at all time high.

@Adam Dong The thumb "tingling" that I was referring to as present a lot on Keto will go away with a normal diet. However, I actually have no ideas on about 7 months on Keto with taking my pills, no a single flare. Just the tingling being present a lot.
