Remote mental health services have been around for a while, long before the pandemic. So, we've had plenty of time to study how well they work, and there are some encouraging findings.

I work in adult mental health services, primarily psychosis patients. The irony of having to ask someone things along the lines of: "Are you having any paranoid, strange or intrusive thoughts? Do you feel like people are watching you, can you hear or see things that others can't? Any command thoughts coming from your phone, computer or the TV? Do you feel like people monitoring you through technology or hidden cameras..." ...all whilst saying this over a video call...

I've been seeing my therapist over video calls for many years, way before covid. They've always felt the same as in person, though I don't know if the phone would work as well for me. It helps to see the person's face.

From personal experience I'll say, that like with many things relating to mental health it has a lot to do with attitude. If the predisposition of the patient towards teletherapy is negative (like mine) it's unlikely for the session to feel like it "accomplished" something, unlike most in-person therapy sessions. Another factor for me personally, is that the meeting in person does allow the patient to feel like they're in a private space and can discuss anything and everything. For people who live with their abusers, or even just friends/family it seems to me like it HAS to have a negative impact on the honesty in communication and the general willingness to divulge information. I've told my therapist things about myself I've never told anyone else in my life, there is no way I'd divulge that information if we weren't in a room together with the utter certainty that other people can't hear us.

I've worked in mental health for many years, with severely mentally ill clients, the kind that can be paranoid about technology. Telehealth did not work during the pandemic for some of our clients, but elevated them instead. We had to make special accommodations for certain clients to get the care they need.

Having transitioned from in-person therapy to teletherapy I can say all of this rings pretty true to me. I will also say that when I started therapy I was in a much worse place than I am now, and having someone to hug every week felt like water in the desert. Any therapy is better than no therapy, though, and it's important to get it however you can if you need it. (Edit: any therapy where you and your therapist have a good rapport. Bad therapy, where you and your provider are at odds, is probably worse than no therapy.)

yes its a great option for lots (maybe majority) of people and it should stick around as an optional alternative forever. BUT a lot of people dealing with mental health really struggle with detachment and de-personalization and walking into the office, talking to someone in person , holding attention and practicing eye contact is very important part of treatment . the added tangibility of in-person visits is so crucial especially for people most in need of help eg long term therapy/psychiatrists/getting effective precriptions.

It has been my experience during the pandemic that many mental health professionals are very poorly skilled at setting up an IT solution even as simple as videoconferencing. Only one of my four mental health professionals figured it out and that's only because she jumped on it as soon as COVID-restrictions came into play and went through a lot of trial and error. I am very grateful she did though.

Yup, that's my main concern with anything teleconference: people and organizations are utterly horrible with computers and putting any amount of trust in them doing it right and in a secure way has time and time again been shown to be naive.

i think telehealth therapy has a long way to go, there really has to be a standard to set for WHERE and how these appointments go, many times have I had to basically cancel important telehealth appointments for both physical and mental health all because the therapists wants you to use their Own specific weird buggy app, and isnt guaranteed to always be a suerfire thing. its way too inconsistent and while i think telehealth is very important for improving mental health, we need to make it less complicated for people as it really just becomes more of a headache sometimes than a blessing. making a standard where most telehealth therapy sessions happen is so important, and therapists should also be willing to bend to the will of their patients to help make therapy more accessible for them; after all, we're paying for it!

There are some great national trainings on doing teletherapy, and a good understanding of the ACA code of ethics, section H has some great info about how to make it a safe, therapeutic environment and prevent a lot of the bad experiences people have had. Overall, it can be a great way to provide services to people with barriers to in-person, if done ethically.

I have been doing teletherapy (or "telehealth" as my clinic refers to it) ever since the outbreak and I have found it effective enough. Since I do it as part of managing anxiety and depression (and a bit of physical care), I can't say how well it works for those with other conditions though. The main downside is the frequent technical issues that tend to happen with the web client my clinic uses, and even then we can always resort to just talking over the phone if necessary. I could understand the issue of distractions, but they don't happen too often in my experience, and the fact that these sessions can be done at home instead of in-person has done wonders for making me feel comfortable and less stressed during my appointments (though that's mostly associated with traveling to the clinic, rather than being uncomfortable in the clinic).

I delayed getting help because it was so hard to get in person care during the pandemic and my providers took a long time to get remote options. I finally made the decision to meet with an online provider in my city and I’m so glad! The flexibility and comfort of not having to leave my safe space is amazing. And this doesn’t lead to my entire day being ruined depending on what happens in that counselors/psychiatrists office. Great video and topic!

I have developed some troubling issues, and as we had a good rapport, my GP agrees that this would be an option. I did try some psychs where I live, but I didn't have the same connection with them. I'm not belittling them, they seemed to know what they were doing. It was only recently that I mentioned my former psych to my GP and we found out that she does online sessions. Yay! So this video is quite timely. Thank you for adding to the feeling of relief. Cheers! :)

telehealth was actually a HUGE improvement for my therapy; i was seeing two therapists (a general therapist and one who pre-covid led a DBT group i was part of), and as a disabled person living with chronic pain and fibro myalgia, starting video call sessions increased my ability to attend my appointments, which meant my appointments were more regular and i was getting more out of them. my chronic pain or even just my anxiety disorder sometimes prevented me from leaving the house, but now that my sessions are IN the house, all i need to do is wake up and turn on my computer. this is why i'm continuing to do telehealth even now that my DBT therapist is doing in-person groups again, and we're both vaccinated. i hope that telehealth continues to be an option more widely, because for disabled folks like myself, it really does make therapy more accessible than it's ever been. [also, if the IBS strikes, i can drop that deuce right in my own toilet, which is a massive improvement over having to flee to my therapist's public restroom i must say!]
@Miss-Anne-Thrope

I'm currently doing DBT therapy online and in many ways I think it's better to have therapy online. I don't have to worry about transport to sessions, I don't get sensory overload, I don't get stressed navigating buildings, I don't have to make eye contact with people and if I get upset I can turn the camera off so that no one can see me. Plus I love being able to have my rabbits with me! 😊 I'll also be able to access other therapies that aren't run in my area.

I haven't really done enough video chats to know how I handle that, but I do get a lot of anxiety over phone calls. Of course, having to go to any kind of in-person appointment (health or otherwise) without someone to take the lead in navigating the building or help with paperwork is pretty nerve-wracking for me, too. I am in general more comfortable with text-based communication than phone calls, maybe because of the ability to proofread what I'm saying, and also no expectation from the other person of a completely instantaneous response.

I'm considering starting therapy to pursue a diagnosis, and I specifically want in person therapy because I think that my body language may help inform that diagnosis. Also, I live with my partner and while I do talk about this stuff all the time with him, being constantly aware that he may be able to overhear such an intimate medical appointment would make it harder to communicate with the therapist, because I'd be thinking about not just how the person I was talking to would respond, but about how he would respond too. That's just something I don't want to mentally keep track of

I can't drive and I live in a rural area, I don't have a bus route near me and the closest office I could see someone in person is a 45 minute drive away. Without telehealth services I had felt like too much of a burden on my support system to seek help. I also find it personally easier to talk to someone over the phone than face to face bc I don't feel the need to read micro body language and self correct, the other person is "less real," if that makes sense.

The answer to this question would be just as varied as methods of treatment, because everyone is different (I haven't yet watched the video, so I'm hoping this is the conclusion they come to). It would be a horribly ineffective method for me, for example, because webcams make me incredibly anxious. In addition, I really need to be outside my home and in another space. Otherwise, like the Cancerian I am, I'd just hide in my shell. But even though it's not for me, I'm so thrilled that it exists and is constantly improving. As others have mentioned, it's fantastic for those with physical limitations. I can't comment on the financial aspect, because for my broke ass it's still wildly expensive. Anyway, I think this is a really misleading question, because there is no "correct" answer.

Its the reason I didn't do myself in last year, so yes, it's effective at least for some people. Especially since I'm too disabled to drive, so I have a hard time getting to appointments in the first place. The only problem I had was I had to make sure my mother was at work before I felt comfortable enough to speak, since I would have hell to pay if she knew the reason I've been wanting to die. At one point, my mom took a day off and I had to type for my therapist so that I wouldn't be overheard.

I did over-the-phone therapy. Only two sessions. The first session was the assessment, which went as expected. The second session went something like this: Therapist: Hello, how are you doing today? Me: I'm good, thanks. How are you? Therapist: Oh that's great! We can end the session now if you like. I was completely taken aback by this, and said sure to ending the session, but what I actually should have said "Wait... no... 'I'm good, thanks. How are you?' is just my programmed automatic response because I work with the public, and I'm actually very much not good, and that's the reason I didn't cancel this appointment....". Yes, I realize it was "my fault" for ending the call after 30 seconds of talking, but I feel like just because I tell you I'm good in a very convincing way, probably doesn't mean I want to just jump ship, and my automatic agreement is also me being programmed to respond to serve the public. Anyways, I haven't had any more sessions with her, or anyone else, ever since.

I've done online therapy before and can I say that most therapists' lack of technological skills is unnerving. They don't bother to buy a decent mic, use the laptop cam, or worse - I had one therapist connect using her ipad, I could only see her face close-up (not even her hair!) and because she was holding it against a wall (I assume) it kept falling over, and still she didn't bother to buy some decent equipment. I've no doubt online therapy can be effective if done correctly, but I'm yet to find a decent online therapist myself.

I lost my therapist. and tried another. She was really dismissive of some things I said, and prescribed pills after 10 minutes. Our next appointment got rescheduled because she was ill. Then lockdown. The office called to set up teletherapy and said "On your smartphone...." I explained I had a laptop and a tablet but no smartphone. So the person said the therapist would call. Nope. I called before the office closed, and complained. They said she would call the next day. No call. Needless to say, I no longer have a therapist and left a scathing review on Yelp.

These results are extremely skewed. Many of the ones that didn't come on premise were told not to, and to try doing online. So they were most likely under the impression they were not supposed to be going in anymore. It's really bad....

I hate phone calls so it would be terrible for me. Also, I'd be paranoid that my neighbors could hear me. The thing about going to a therapist's room is that they usually have two doors and music playing in the reception so you can rest assured that no one is hearing you. We don't have that structure at home. Also, I live in an area where internet isn't that great... can you imagine your screen freezing just as you tell something big to your therapist? Nah, I need to go there, in a room that was projected for therapy. And have actual human contact. Teletherapy might work but it's not for everyone and I doubt it is as good as in person therapy.

@@amymeyers9682 I guess it depends on where you live? I live in a small town and the houses only have like 10cm of space between each other. So depending on where I am in the house I think my neighbors would hear unless I wisper. I think I'd have to do the call on the bathroom because it's the most soundproof place.

Except when technology doesn't work and you're still charged for a full session that didn't happen. Except when your therapist's kids make regular appearances, at her request, to bring her things like tea and coffee. Except when you don't feel comfortable talking about your issues in your house because the walls are thin and others could overhear you. Lots of negatives in my experience. I stopped going.

So sorry to hear that. First, it sounds like you may need a different therapist. Second, is there any way you could just type your questions and responses. Please don't give up. You need help. Maybe check online for somewhere that deals with abuse cases. They are usually more discreet. My heart goes out to you. Take care.
