Perhaps Grey has been taking the medication for months, or even a couple of years, but still had the hallucinations and dreams. This leads me to believe that Grey most likely saw everyone as their terrifying dream counterparts, the dreams he's been having, and he may have killed most of the sprunki for this specific reason.
